婴儿先天性鼻泪管阻塞泪道探通术并发症的临床分析

摘　　要：目的分析婴儿先天性鼻泪管阻塞泪道探通术并发症发生的原因和治疗。方法回顾性分析东莞市寮步医院2009年至2018年的先天性鼻泪管阻塞患儿(1~11个月龄),行泪道探通手术后发生并发症者124眼的临床资料。结果本研究共772眼泪道探通术中12眼(1.55%)发生假道,112眼(14.51%)发生泪点出血。并发症发生率随月龄增加而增加,3月龄以下患儿无出血发生。操作者熟练程度与假道的发生有关(χ^2=12.00,P=0.001),与出血的发生无关(χ^2=0.562,P=0.453),探通并发症与感染无关(P>0.05)。结论先天性鼻泪管阻塞患儿,在2~3个月龄时行泪道探通手术并发症少,规范操作、技术熟练,可有效减少并发症的发生。Objective To analyze the causes and the clinical preventive measures of the complications of lacrimal duct probing for obstruction of nasolacrimal duct of infants.Methods The clinical data of 124 eyes with complications after infants,aged 1-11 months,with congenital nasolacrimal duct obstructions who underwent lacrimal duct probing from 2009 to 2018 in Liaobu Hospital of Dongguan were retrospectively analyzed.Results There were 12 eyes(1.55%)with false passage and 112 eyes(14.51%)with lacrimal punctum hemorrhage among 772 eyes with lacrimal duct probing.The incidence of complications increased with the increase of month age,and no bleeding occurred in infants below 3 months.Operator proficiency was related to the occurrence of false passage(χ^2=12.000,P=0.001),but not related to the occurrence of bleeding(χ^2=0.562,P=0.453).Probe complications were not related to infection(P>0.05).Conclusion The complications after lacrimal duct probing in infants with congenital nasolacrimal duct obstruction aged 2-3 months is less.Standardized operation and skilled technique can effectively reduce the incidence of complications.
