摘　　要：目的探讨血清甲胎蛋白(AFP)、高尔基体糖蛋白73(GP73)及miR-221联合检测对于原发性肝细胞癌的诊断价值。方法采用电化学发光法检测50例原发性肝细胞癌(肝癌组)及50例肝炎肝硬化(肝硬化组)患者血清AFP值,同时采用酶联免疫法检测血清GP73、PCR荧光探针法检测miR-221,50例健康者作为对照组。结果AFP、GP73及miR-221联合检测对原发性肝细胞癌诊断的敏感性分别为66%、64%及74%,特异性分别为78%、80%及70%;三者联合检测对原发性肝细胞癌诊断的敏感性为86%,有统计学差异(P<0.05),特异性为69%,无统计学差异。结论AFP、GP73及miR-221联合检测可提高原发性肝细胞癌诊断的敏感性,有利于早期原发性肝细胞癌诊断效能。Objective To investigate the diagnostic value of serum alpha-fetoprotein(AFP),Golgi glycoprotein 73(GP73)and miR-221 in primary hepatocellular carcinoma.Methods AFP in 50 patients of primary hepatocellular carcinoma or hepatic cirrhosis were detected by electrochemiluminescence assay.Meanwhile,GP73 and miR-221 were detected by enzyme linked immunosorbent assay(EL1 SA).50 healthy people served as the control group.Results The sensitivity of AFP,GP73 and miR-221 in the diagnosis of primary hepatocellular carcinoma was 66%,64%and 74%respectively,and the specificity was 78%,80%and 70%respectively.The sensitivity and specificity of the combined detection were 86%(P<0.05)and 69%respectively.Conclusion The combined detection of AFP,GP73 and miR-221 can improve the sensitivity of diagnosis of primary hepatocellular carcinoma.It is helpful in diagnosis of primary liver cancer.
