Survivin和Aurora-b表达对喉癌诊断及病情评估的临床意义

摘　　要：目的探讨生存素(Survivin)和Aurora-b蛋白表达对喉癌诊断及病情评估的临床意义。方法选取78例喉癌患者,采用手术治疗,取喉癌石蜡包埋标本;同时取20例喉室息肉或声带息肉的患者的正常喉增殖期组织石蜡包埋标本,检测Survivin和Aurora-b蛋白在不同组织中的表达情况。对喉癌患者进行电话或门诊复诊随访。结果经检测,Survivin和Aurora-b在喉癌组织中表达的阳性率分别为75.6%、60.3%,显著高于在正常组织中的阳性表达率分别为10.0%、5.0%(P<0.01)。Survivin、Aurora-b在临床分期Ⅳ、Ⅲ期的喉癌中阳性率显著高于Ⅱ、Ⅰ(P<0.05);Survivin、Aurora-b在高分化的阳性率高于低分化的阳性率(P<0.05);Survivin和Aurora-b在淋巴结转移患者组的阳性率低于非淋巴结转移患者组的阳性率(P<0.05);声门下型的Survivin和Aurora-b阳性率显著高于声门上型和跨声门型(P<0.05)。Survivin和Aurora-b低表达者术后5年总生存率、术后生存时间均显著高(长)于高表达者(P<0.01)。结论检测Survivin和Aurora-b表达可判断喉癌的临床分期、分化情况等,且对评价患者预后意义重大。Objective To analyze the significance of the expression of Survivin and Aurora-b in the diagnosis and evaluation of laryngeal cancer.Methods 78 cases of laryngeal cancer paraffin-embedded specimens were obtained,which were diagnosed as laryngeal carcinoma by pathology.In the patients diagnosed with laryngeal polyps or vocal cord polyps,20 specimens of normal laryngeal proliferative tissue were embedded in paraffin-embedded specimens.The detection procedure was carried out according to the instructions.At the same time,all patients were followed up by telephone or clinic.Results The positive rates of Survivin and Aurora-b expression in laryngeal carcinoma were 75.6%and 60.3%,respectively,and the positive expression rates in normal tissues were 10.0%and 5.0%,respectively.The positive rate of Survivin and Aurora-b expression in laryngeal carcinoma was significantly higher than that in normal tissues,and the difference was statistically significant(P<0.01).The positive rate of Survivin and Aurora-b in laryngeal carcinoma of stageⅣandⅢin clinical stage was significantly higher thanⅡandⅠ(P<0.05).The positive rate of Survivin and Aurora-b in high differentiation is higher than that in low differentiation(P<0.05).The positive rate of Survivin and Aurora-b in patients with lymph node metastasis was higher than that in patients with non-lymph node metastasis(P<0.05).The positive rates of subglottic Survivin and Aurora-b were significantly higher than those of supraglottic and transglottic(P<0.05).The 5-year overall survival rate and postoperative survival time of patients with low Survivin and Aurora-b expression were significantly higher(longer)than those with high expression,and the difference was statistically significant(P<0.01).Conclusion The detection of Survivin and Aurora-b expression can determine the clinical stage and differentiation of laryngeal carcinoma,and it is of great significance for evaluating the prognosis of patients.
