甲状腺乳头状癌病理特征与BRAF V600E基因突变的相关性

摘　　要：目的探讨与研究甲状腺乳头状癌病理特征与BRAF V600E基因突变的相关性。方法选择甲状腺外科手术治疗病例共184例,其中甲状腺乳头状癌患者60例(甲状腺癌组),良性甲状腺肿124例(甲状腺肿组)。检测所有患者外周血分子标志物BRAF V600E基因突变情况,并分析其与病理特征的相关性。结果甲状腺癌组患者的BRAF V600E基因突变率为55.0%,显著高于甲状腺肿组的6.5%(P<0.05)。在甲状腺癌组60例患者中,随着病理分期增加、分化程度降低、淋巴结转移与包膜浸润,BRAF V600E基因突变率显著增加,对比差异有统计学意义(P<0.05)。多因素logistic回归分析显示临床分期(Ⅱ期/Ⅲ期)、分化程度(低分化)、淋巴结转移、包膜浸润都为影响患者BRAF V600E基因突变率的因素(P<0.05)。结论甲状腺乳头状癌患者多伴随有BRAF V600E基因突变,且与患者的病理特征显著相关,可用于辅助判断患者的病情。Objective To investigate the relationship between pathological features of papillary thyroid carcinoma and BRAF V600E gene mutation.Methods 184 patients of thyroid surgery were selected that included 60 patients of thyroid papillary carcinoma(thyroid cancer group)and 124 patients of benign goiter(thyroid group).Detect the mutation of BRAF V600E gene in peripheral blood molecular markers of all patients.Results The BRAF V600E gene mutation rates was 55.0%in the thyroid cancer group,which was significantly higher than 6.5%in the goiter group(P<0.05).In the 60 patients with thyroid cancer,with the increase of pathological stage,differentiation degree,lymph node metastasis and capsule infiltration,the mutation rate of BRAF V600E gene increased significantly,and the difference was statistically significant(P<0.05).Multivariate logistic regression analysis of clinical stage(stage II/III),degree of differentiation(low differentiation),lymph node metastasis,and capsule infiltration were the factors affecting the BRAF V600E gene mutation rates(P<0.05).Conclusion Patients with papillary thyroid carcinoma are often associated with BRAF V600E gene mutation,which is significantly correlated to the pathological characteristics of the patient and can be used to assist in the judgment of the patient.
