摘 要:目的探讨血管内皮生长因子(VEGF)、基质金属蛋白酶-7(MMP-7)、MMP-9及二肽酶1(DPEP1)在结肠癌并发肾功能障碍中的检测意义。方法选择60例结肠癌患者为研究对象,应用免疫组化SP法检测其癌组织及癌旁组织中VEGF、MMP-7、MMP-9及DPEP1阳性率,另选取同期接受体格检查的60例健康个体为对照组,对比3组患者VEGF、MMP-7、MMP-9及DPEP1阳性率差异,分析上述因子与结肠癌病理学指标的关系,将60例结肠癌患者按照是否存在肾功能障碍区分为障碍组(21例)和非障碍组(39例),对比两组患者上述因子阳性率差异。结果癌组织中VEGF、MMP-7、MMP-9及DPEP1阳性率高于癌旁组织(P<0.05),癌旁组织上述因子阳性率高于对照组(P<0.05);中低分化、淋巴结转移、临床晚期结肠癌患者VEGF、MMP-7、MMP-9及DPEP1阳性率明显升高(P<0.05);存在肾功能障碍的结肠癌患者VEGF、MMP-7、MMP-9及DPEP1阳性率明显高于非障碍患者(P<0.05)。结论VEGF、MMP-7、MMP-9及DPEP1在检测结肠癌病情中具有一定应用价值,其水平与结肠癌分化程度、组织浸润深度、淋巴结转移有关,同时并发肾功能障碍患者上述因子阳性率明显上升,可通过对上述因子的监测来调整治疗方案。Objective To explore the significance of detection of vascular endothelial growth factor(VEGF),matrix metalloproteinase-7(MMP-7),MMP-9 and dipeptidase 1(DPEP1)in colon cancer complicated with renal dysfunction.Methods60 colon cancer patients who were treated in the study.The immunohistochemical SP method was used to detect the positive rate of VEGF,MMP-7 and MMP-9 and DPEP1 in cancer tissues and adjacent tissues.Another 60 healthy individuals who underwent physical examination as the control group,compared the difference of VEGF,MMP-7,MMP-9 and DPEP1 positive rate in the three groups,and analyzed the above factors and colon cancer The relationship between the physical indicators,and finally 60 patients with colon cancer were divided into the disorder group(21 cases)and the non-disorder group(39 cases)according to the presence or absence of renal dysfunction.Results Contrast showed that the positive rate of VEGF,MMP-7,MMP-9 and DPEP1 in cancer tissues was higher than that of adjacent tissues(P<0.05),and the positive rate of the above-mentioned factors in adjacent tissues was higher than that of the control group(P<0.05).The analysis found that the positive rates of VEGF,MMP-7,MMP-9 and DPEP1 were significantly increased in patients with moderate to poor differentiation,lymph node metastasis and advanced colon cancer(P<0.05).Comparison showed that The positive rates of VEGF,MMP-7,MMP-9 and DPEP1 in patients with renal dysfunction were significantly higher than those of in non-obstructive patients(P<0.05).Conclusion VEGF,MMP-7,MMP-9 and DPEP1 had certain application value in the detection of colon cancer,and their levels were related to colon cancer differentiation,tissue infiltration depth,lymph node metastasis,and the positive rate of these factors in patients with renal dysfunction.At the same time,the positive rate of the above factors in patients with concurrent renal dysfunction increased significantly,and the treatment plan could be adjusted by monitoring the above factors.
