血清CA153和CA125表达对乳腺癌患者预后的判断价值

摘　　要：目的探讨乳腺癌患者血清糖蛋白抗原(carbohydrate antigen,CA)153和CA125表达对预后的判断价值。方法选择乳腺癌患者78例作为研究对象,记录患者入院时的临床资料,检测血清CA153、CA125表达水平。所有患者随访6个月,调查预后死亡情况并进行相关性分析。结果在78例患者中,随访6个月,生存60例,死亡18例,死亡率为23.1%。存活组与死亡组的性别、年龄、体重指数、WBC、PLT、ALT、TBA、Cr等对比差异无统计学意义(P>0.05),两组的临床分期、组织学分化、病理类型等对比差异有统计学意义(P<0.05)。死亡组的血清CA153、CA125含量显著高于存活组(P<0.05)。直线相关分析显示:乳腺癌患者预后与CA153、CA125、临床分期、组织学分化、病理类型有显著正相关性(P<0.05);血清CA153、CA125判断乳腺癌患者预后死亡的受试者工作特征曲线(receiver operating characteristic curve,ROC曲线)下总面积为0.890、0.873(95%CI值为1.263~1.917,1.093~1.556)。结论中晚期乳腺癌患者的预后死亡率比较高,血清CA153、CA125值与患者预后显著相关,且能对患者预后进行有效判断。Objective To investigate the clinical value of serum glycoprotein antigen(CA)153 and CA125 in breast cancer patients for prognosis.Methods 78 patients with breast cancer were selected as subjects.The clinical data of patients were recorded,and the serum CA153 and CA125 expression levels were detected.All patients were followed up for 6 months to investigate the prognosis of death and correlation analysis.Results Among 78 patients,followed up for 6 months,there were 60 patients survived,18 patients were died,and the mortality rate was 23.1%.There were no significant differences in gender,age,BMI,WBC,PLT,ALT,TBA,Cr compared between the survival group and the death group(P>0.05).The clinical stage,histological differentiation and pathological type of the 2 groups compared were statistically significant difference(P<0.05).The serum levels of serum CA153 and CA125 in the death group were significantly higher than those in the survival group(P<0.05).Linear correlation analysis showed that the prognosis of breast cancer patients was significantly positively correlated with CA153,CA125,clinical stage,histological differentiation and pathological type(P<0.05).The total area under the receiver operating characteristic curve(ROC curve)of serum CA153 and CA125 were used to judge the prognosis were 0.890,0.873(95%CI values were 1.263~1.917,1.093~1.556).Conclusion The prognosis mortality of patients with advanced breast cancer is relatively high.The serum CA153 and CA125 values are significantly correlated with the prognosis of patients,and can effectively judge the prognosis of patients.
