LNSRH与LRH治疗70例早期宫颈癌的远期疗效分析

摘　　要：目的探讨LNSRH与LRH对早期宫颈癌患者的远期疗效的影响。方法将70例早期宫颈癌患者根据不同术式分为对照组和研究组2组,每组35例,对照组患者接受腹腔镜下广泛性子宫切除术(LRH)治疗,研究组患者接受腹腔镜下保留盆腔自主神经广泛性子宫切除术(LNSRH)治疗,比较2组患者的手术情况、膀胱功能恢复情况、并发症发生率及生存时间。结果2组患者的手术时间、术中出血量、淋巴结清扫数、阴道及宫旁切除长度比较无显著性差异(P>0.05),2组尿失禁的发生率比较无统计学差异(P>0.05),研究组膀胱功能障碍、尿潴留、腹压排尿的发生率及保留尿管时间均显著少于对照组(P<0.05),2组患者术后并发症发生率比较无统计学差异(P>0.05),2组患者治疗后1年、2年生存率及中位生存时间比较无统计学差异(P>0.05)。结论LNSRH与LRH在对早期宫颈癌患者的治疗中的手术情况、术后并发症及生存时间相当,但LNSRH可有效保护患者的膀胱功能,对提高患者的生活质量具有重要意义。Objective To explore long-term efficacy effect of LNSRH and LRH in 70 patients with early cervical cancer.Methods 70 patients with early cervical cancer were selected as research object and conducted retrospective study,according to different procedures,the subjects were divided into the control group and the study group and each group of 35 cases.The control group were received laparoscopic extensive hysterectomy(LRH)treatment,the study group were received laparoscopic preservation of pelvic autonomic extensive hysterectomy(LNSRH)treatment.Surgery,recovery of bladder function,complication rate and survival time were compared the 2 groups.Results There were no significant differences in the operation time,intraoperative blood loss,lymph node dissection,vaginal and parametrial resection length between the 2 groups(P>0.05).There was no significant difference in the incidence of urinary incontinence between the 2 groups(P>0.05).The incidence of bladder dysfunction,urinary retention,abdominal pressure urination and preserving catheter time were significantly lower in the study group than in the control group(P<0.05).There was no significant difference in the incidence of postoperative complications between the 2 groups(P>0.05).There was no significant difference in the 1 year,2 years survival rate,and median survival between the 2 groups(P>0.05).Conclusion LNSRH and LRH were equivalent in the operation,postoperative complications and survival time of patients with early cervical cancer,but LNSRH could effectively protect the bladder function,which was of great significance to improve the quality of life of patients.
