基于关联规则探讨杨晋翔教授治疗慢性萎缩性胃炎的用药规律

摘　　要：目的基于关联规则探讨杨晋翔教授治疗慢性萎缩性胃炎的用药规律。方法收集2018年5月1日—2019年5月1日就诊于北京中医药大学第三附属医院杨晋翔教授门诊具有明确疗效的120例慢性萎缩性胃炎患者的首诊处方,建立数据库,分别使用IBMSPSSStatistics23.0及IBMSPSSModeler18.0对处方药物进行频数统计分析及关联规则分析。结果通过对120例患者的中药处方进行分析,总结出杨晋翔教授治疗慢性萎缩性胃炎频数较高的10味药是徐长卿、青蒿、预知子、白芍、大腹皮、石见穿、香橼、茵陈、佛手和茯苓;同时挖掘出5个置信度为1的核心药物组合,衍化得到1个核心处方。结论杨晋翔教授治疗慢性萎缩性胃炎善于从肝、胃、脾综合论治,尤擅从肝论治,主要使用以疏肝理气为主的药物,善用辛香之品,注重寒温并用、辨证与辨症相结合。Objective To explore the medication regularity of Professor Yang Jinxing in the treatment of atrophic gastritis based on association rules.Methods The first diagnosis prescriptions of definite efficacy of 120 patients with chronic atrophic gastritis who were diagnosed at Outpatient Department of Professor Yang Jinxiang of The Third Affiliated Hospital of Beijing University of Chinese Medicine from May 1,2018 to May 1,2019 were collected,database was established.IBM SPSS Statistics 23 and IBM SPSS Modeler 18 were used separately to conduct frequency statistics analysis and association rule analysis on prescriptions.Results By conducting analysis on traditional Chinese medicine prescriptions of 120 patients,10 categories of medications with high frequency in treating atrophic gastritis by Professor Yang were summarized:Cynanchum paniculatum(Bunge)Kitagawa,Sweet wormwood,Akebia Fruit,Radix Paeoniae Alba,Pericarpium Arecae,Salvia Chinensis,Citron,Oriental Wormwood,Fingered Citron and Fuling.At the same time,5 core medication combinations with confidence interval as 1 were excavated.One core formula was derived.Conclusion Professor Yang is good at conducting comprehensive therapy from liver,gastry and spleen in the aspect of treating atrophic gastriti,especially good at conducting therapy from liver.He mainly uses drugs for soothing the liver and regulating qi,makes good use of spicy products,and pays attention to the combination of cold and warm,syndrome differentiation and treatment.
