冠状动脉支架植入术后行肺癌根治术的可行性研究

摘　　要：目的研究冠状动脉支架植入术后行肺癌根治术的可行性,为临床应用提供参考。方法回顾性分析于河南省胸科医院2018年4月—2019年3月行肺癌根治术的65例肺癌合并冠脉狭窄患者临床资料,根据其术前治疗方式分为对照组(n=31)与观察组(n=34)。对照组术前经常规抗凝治疗,观察组术前经冠状动脉支架植入术治疗,比较两组肺癌根治术中手术相关指标及术后并发症发生率。结果观察组手术用时、术中出血量对比,差异无统计意义(P>0.05);观察组术后拔管时间、术后引流量及总住院时间较对照组高(P<0.05);观察组术后并发症总发生率较对照组略低,但差异无统计意义(P>0.05)。结论冠状动脉支架植入术后可进行肺癌根治术治疗,但手术前后需合理应用抗凝药,并做好围术期相关准备。Objective To study the feasibility of radical resection of lung cancer after coronary stent implantation and to provide reference for clinical application.Methods The clinical data of 65 patients with lung cancer complicated with coronary stenosis who underwent radical resection of lung cancer in Henan chest hospital from April 2018 to March 2019 were analyzed retrospectively,the patients were divided into control group(n=31)and observation group(n=34).The patients in the control group were treated with routine anticoagulation before operation,and the patients in the observation group were treated with coronary stent implantation before operation.The operative parameters and postoperative complications were compared between the two groups.Results There was no significant difference in operation time and blood loss in the observation group(P>0.05),the decannulation time,drainage volume and total hospitalization time were higher in the observation group than those in the control group(P<0.05);The total incidence of postoperative complications in the observation group was slightly lower than that in the control group,but there was no significant difference(P>0.05).Conclusion Radical resection of lung cancer can be performed after coronary stent implantation,but anticoagulant should be used rationally before and after operation,and relevant preparations should be made during perioperative period.
