摘 要:目的研究JAK2V617F突变结果呈阳性骨髓增殖性肿瘤患者实施骨髓细胞形态观察与骨髓活检联合检查的检测作用。方法择取2018年4月—2019年4月37例信阳市固始县信合医院纳入的JAK2V617F突变结果呈阳性骨髓增殖性肿瘤患者,另选取非JAK2V617F突变结果呈阳性骨髓增殖性肿瘤患者13例作为对照,全体研究对象均实施骨髓活检与骨髓细胞形态观察,讨论患者的检查诊断结果,对比骨髓活检、骨髓细胞形态观察与骨髓活检联合检查诊断准确性、JAK2V617F突变结果呈阳性骨髓增殖性肿瘤患者病态造血结果。结果骨髓细胞形态观察与骨髓活检联合检查诊断结果灵敏度(94.59%)、特异度(92.31%)、准确性(94.00%)均明显高于骨髓活检(78.38%、30.77%、66.00%),两者差异显著(P<0.05)。骨髓细胞形态观察与骨髓活检联合检查红系造血结果(35.14%、40.54%)具有一致性(P>0.05),JAK2V617F突变结果呈阳性骨髓增殖性肿瘤患者骨髓细胞形态观察与骨髓活检联合检查骨髓粒系造血结果(24.32%)低于骨髓活检(51.35%),与JAK2V617F突变结果呈阳性骨髓增殖性肿瘤患者骨髓活检(13.51%)相比,骨髓细胞形态观察与骨髓活检联合检查巨核系造血结果(35.14%)较高,其间具有显著差异(P<0.05)。结论JAK2V617F突变结果呈阳性骨髓增殖性肿瘤患者通过骨髓细胞形态观察与骨髓活检联合检查效果确切,能够提升其诊断准确率。Objective To study the effect of combined examination of bone marrow cell morphology and bone marrow biopsy on JAK2V617F mutation-positive patients with myeloproliferative tumor.Methods From April 2018 to April 2019,37 patients with JAK2V617F mutation were selected from Gushi county,Xinyang,in addition,13 patients with non-JAK2V617F mutation positive myeloproliferative tumor were selected as the control group.All subjects underwent bone marrow biopsy and morphological observation of bone marrow cells,the diagnostic accuracy of combined examination of bone marrow biopsy,morphological observation of bone marrow cells and bone marrow biopsy,and abnormal hematopoiesis in patients with positive JAK2V617F mutation were compared.Results the sensitivity(94.59%),specificity(92.31%)and accuracy(94.00%)of bone marrow cell morphology and bone marrow biopsy were significantly higher than those of bone marrow biopsy(78.38%,30.77%,66.00%)(P<0.05).The results of hematopoiesis(35.14%,40.54%)were consistent with that of bone marrow biopsy(P>0.05),the results of JAK2V617F mutation(24.32%)were lower than that of bone marrow biopsy(51.35%)in the patients with myeloproliferative tumor,compared with JAK2V617F mutation positive patients(13.51%),bone marrow cell morphology and bone marrow biopsy combined with megakaryocytic hematopoiesis(35.14%)were significantly higher(P<0.05).Conclusion Combined examination of morphology of bone marrow cells and bone marrow biopsy is effective in diagnosis of JAK2V617F positive myeloproliferative tumor.
