不同病理类型外周T细胞淋巴瘤患者中枢神经系统复发情况探究

摘　　要：目的探究不同病理类型外周T细胞淋巴瘤(PTCL)患者中枢神经系统(CNS)复发的情况。方法收集450例PTCL患者的临床资料,包括年龄、性别、病理类型、化疗方案和化疗效果。比较不同病理类型PTCL患者的年龄、性别、化疗方案、化疗效果以及不同病理类型和化疗效果的PTCL患者CNS复发情况。结果 450例PTCL患者中,13例患者出现CNS复发,其中,间变性淋巴瘤激酶(ALK)阳性的间变性大细胞性淋巴瘤(ALK+ALCL)患者5例。不同病理类型PTCL患者接受不同化疗方案的比例比较,差异均有统计学意义(P<0.05)。不同病理类型PTCL患者获得完全缓解(CR)的比例比较,差异有统计学意义(P<0.01)。其中,非特指性PTCL(PTCL-NOS)患者获得CR的比例最低,ALK+ALCL患者获得CR的比例最高。13例患者出现CNS复发的中位时间为7个月,5年CNS复发率为2.89%(13/450)。不同化疗效果的PTCL患者的CNS复发率比较,差异无统计学意义(P>0.05)。结论不同病理类型PTCL患者的CNS复发率不同,ALK+ALCL患者CNS的复发率更高。Objective To explore the central nervous system(CNS) relapse in patients with peripheral T-cell lymphoma(PTCL) of different pathological types.Method The information(age,sex,pathological type,chemotherapy regimen and chemotherapy effect) of 450 PTCL patients were collected.The age,sex,chemotherapy regimen and chemotherapy effect in patients with different pathological types,and the CNS relapses of different pathological types and chemotherapy effect were compared.Result There were 13 cases of CNS relapse in 450 patients,with 5 cases of anaplastic lymphoma kinase positive and anaplastic large cell lymphoma(ALK+ALCL)patients.Comparison of the proportions of PTCL patients wpth different pathological types receiving different chemotherapy regiments showed statiotiTally signifirant difers ences(P<0.05).There were significant differences for complete response(CR) rates in PTCL patients with different pathological types(P<0.01),with lowest CR rates for peripheral T cell lymphoma not otherwise specified(PTCL-NOS) patients and highest CR rates for ALK+ALCL patients.CNS recurrence occurred in 13 patients,among whom the median time was 7 months,5-year cumulative CNS relapse rate was 2.89%(13/450).There was no significant difference for CNS relapse in PTCL patients with different chemotherapy effect(P>0.05).Conclusion There are different CNS relapse rates in PTCL patients of dnterent pathological types,with highest rate of CNS relapse m ALK+ALCL patients.
