摘　　要：目的探讨脂蛋白相关磷脂酶A2(Lp-PLA2)、CD147在颈动脉粥样硬化患者血清中的表达及意义。方法选取2018年1月至2019年2月在本院治疗的颈动脉粥样硬化患者87例为观察组,按粥样斑块超声特点分为稳定斑块组(n=42)及不稳定斑块组(n=45);同时选取健康志愿者50例作为对照组,检测血清血清Lp-PLA2和CD147水平,比较各组血清Lp-PLA2和CD147水平差异。采用Pearson检验分析颈动脉粥样硬化患者血清Lp-PLA2、CD147表达的相关性;绘制ROC曲线,评估Lp-PLA2、CD147对颈动脉粥样硬化斑块稳定性的评估价值。结果不稳定斑块组血清Lp-PLA2、血清CD147水平高于稳定斑块组及对照组,差异有统计学意义(P<0.05);稳定斑块组血清Lp-PLA2、CD147水平高于对照组,差异有统计学意义(P<0.05)。相关分析显示,颈动脉粥样硬化患者血浆Lp-PLA2和血清CD147呈正相关(r=0.443,P<0.05);ROC曲线显示,血清Lp-PLA2和CD147联合检测对颈动脉粥样斑块不稳定的AUC值为0.874,高于Lp-PLA2和CD147单独检测AUC值。结论颈动脉粥样硬化斑块患者血清Lp-PLA2、CD147明显升高,Lp-PLA2、CD147联合检测有助于评估斑块的稳定性。Objective To investigate the expression and significance of serum Lp-PLA2 and CD147 in patients with carotid atherosclerosis.Method 87 patients with carotid atherosclerotic stenosis who were treated in our hospital from Jan 2018 to Feb 2019 were divided into the stable plaque group(n=42)and unstable plaque group(n=45)according to ultrasonic characteristics of atherosclerotic plaque.50 healthy volunteers were selected as the control group,The serum levels of Lp-PLA2 and CD147 were detected and compared among three groups.The expresive correlation between Lp-PLA2 and CD147 were analyzed by Pearson.TheROC curve was drawn,and evaluated.Result The serum levels of Lp-PLA2 and CD147 were higher in the unstable plaque group than that in the stable plaque group and control group.The difference was statistically significant(P<0.05),and the levels of Lp-PLA2 and CD147 was higher in the stable plaque group than in the control group,the difference was statistically significant(P<0.05).The correlation analysis showed that the levels of Lp-PLA2 and CD147 in the carotid atherosclerosis patients were positively correlated(r=0.443,P<0.05).The ROC curve showed that the combined detection of serum Lp-PLA2 and CD147 had a change of 0.874 in the AUC of carotid atherosclerotic plaque,which was higher than the AUC value of LpPLA2 and CD147 alone.Conclusion Serum Lp-PLA2 and CD147 in patients with carotid atherosclerotic plaque are significantly increased.The combined detection of Lp-PLA2 and CD147 is helpful to assess the stability of plaque.
