摘 要:目的研究食管癌(EC)癌组织中程序性死亡受体配体1(PD-L1)、波形蛋白(Vim)和锌指结构E-box结合蛋白1(Zeb1)表达水平与放疗敏感性的关系。方法选取2016年10月至2019年10月本院中晚期EC患者106例,行食管肿瘤胃镜活检并将标本进行免疫组化分析,然后采用三维适形调强放疗(IMRT)进行根治性放疗,根据放疗后4周时疗效将患者分为敏感组和耐受组,比较两组免疫组化分析结果并分析各指标与EC放疗敏感性的关系。结果106例EC患者放疗效果显示完全缓解(CR)37例,部分缓解(PR)34例,稳定(SD)23例,进展(PD)为12例,敏感组(CR+PR)癌组织程序性死亡受体配体1(PD-L1)、波形蛋白(Vim)、锌指结构E-box结合蛋白1(Zeb1)和信号传导和转录激活因子3(STAT3)阳性率均低于耐受组,差异有统计学意义(P<0.05);Logistic回归分析显示肿瘤最大径>1.5 cm、分化程度低以及PD-L1、Vim、Zeb1和STAT3阳性是EC放疗敏感性的独立危险因素,差异有统计学意义(P<0.05);Spearman秩相关分析显示,EC患者肿瘤组织PD-L1、Vim和Zeb1阳性率与STAT3均呈明显正相关性,差异有统计学意义(P<0.05)。结论EC肿瘤组织PD-L1、Vim和Zeb1蛋白在放疗耐受患者中阳性表达率更高,且其与放疗敏感相关蛋白STAT3表达水平具有明显正相关性,可作为评估EC放疗敏感性的参考指标。Objective To study the relationship and role mechanism between the expression levels of programmed cell death-ligand 1(PD-L1),Vimentin(Vim)and zinc finger E-box-binding protein 1(Zeb1)in esophageal carcinoma(EC)tissues and radio-sensitivity.Methods From October 2016 to October2019,106 patients with advanced EC in our hospital were selected for esophageal mucosal puncture biopsy,and specimens were analyzed by immunohistochemistry,and the three-dimensional conformal intensity-modulated radiation therapy(IMRT)was used for radical radiotherapy.At 4 weeks after radiotherapy,the patients were divided into the sensitive group and the tolerant group.The immunohistochemical analysis results of the two groups were compared and the relationship between the indicators and the sensitivity of EC radiotherapy was analyzed.Results The radiotherapy effect of 106 EC patients showed complete remission(CR)in 37 cases,partial remission(PR)in 34 cases,stable(SD)in 23 cases,and progression(PD)in 12 cases.The positive rates of Programmed death receptor ligand 1(PD-L1),vimentin(Vim),zinc finger structure E-box binding protein 1(Zeb1)and signal transduction and transcription activator 3(STAT3)in the sensitive group(CR+PR)cancer tissue were lower than those in the tolerance group,the difference was statistically significant(P<0.05).Logistic regression analysis showed that the largest tumor diameter>1.5 cm,low differentiation,and positive PD-L1,Vim,Zeb1 and STAT3 were independent risk factors for EC radiotherapy sensitivity,and the difference was statistically significant(P<0.05).Spearman rank correlation analysis showed that the positive rates of PD-L1,Vim and Zeb1 in tumor tissues of EC patients were significantly positively correlated with STAT3,and the difference was statistically significant(P<0.05).Conclusion The positive expression rate of PD-L1,Vim and Zeb1 proteins in EC tumor tissues is higher in patients with radiotherapy tolerance,and it has a significant positive correlation with the expression level of radiosensitivityrelated
