摘　　要：目的观察耳内镜下经外耳道后壁翻瓣夹层法对鼓膜穿孔患者的修补效果。方法回顾性分析2015年1月至2019年1月在陕西省第四人民医院耳鼻喉科进行耳内镜下经外耳道后壁翻瓣夹层法治疗的52例鼓膜穿孔患者的临床资料。结果52例患者术后愈合率为100%,愈合情况比较良好,无出现外移、内陷等情况;患者手术后一年的气导听阙为(22.75±6.78)dB HL,与手术前的(41.65±10.83)dB HL比较差异有统计学意义(P<0.05);患者术后骨气导差为(10.03±4.67)dB HL,与手术前的(21.32±7.12)dB HL比较差异有统计学意义(P<0.05)。结论采用耳内镜下经外耳道后壁翻瓣夹层法修补鼓膜穿孔,能有效提高患者的鼓膜愈合率,改善患者的听力,在临床治疗中有推广价值。Objective To observe the effect of endoscopic myringoplasty via sandwich method with posterior wall flap of external auditory canal on patients with tympanic membrane perforation.Methods From January 2015 to January 2019,52 patients with tympanic membrane perforation were treated by endoscopic myringoplasty via sandwich method with posterior wall flap of external auditory canal in the Department of Otolaryngology,the Fourth People's Hospital of Shaanxi Province were retrospectively analyzed.Results The healing rate of 52 patients was 100%after operation,and the healing was relatively good,with no outward migration or invagination.The air conduction hearing threshold of the patient after the operation was(22.75±6.78)dB HL,which was significantly lower than(41.65±10.83)dB HL before operation(P<0.05).The air-bone gap was(10.03±4.67)dB HL after operation,which was significantly lower than(21.32±7.12)dB HL before operation(P<0.05).Conclusion Endoscopic myringoplasty via sandwich method with posterior wall flap of external auditory canal can effectively improve the healing rate of the tympanic membrane and improve the hearing of the patients,which has great promotion value in clinical treatment.
