温针灸联合牵引治疗腰椎间盘突出所致坐骨神经痛对致痛物质及TXB2、6-K-PGF1α的影响

摘　　要：目的研究温针灸联合牵引治疗腰椎间盘突出所致坐骨神经痛对致痛物质及TXB2、6-K-PGF1α的影响。方法选取2018年1月至2019年3月岳池县中医医院收治的腰椎间盘突出所致坐骨神经痛患者100例,按随机数字表法分为对照组和试验组,各50例。对照组患者接受常规牵引治疗,试验组患者接受温针灸联合牵引治疗,两组患者均连续治疗30 d。对比两组患者治疗后临床疗效;治疗前后VAS、JOA评分,血浆5-HT、SP、TXB2、6-K-PGF1α水平及T/K比值。结果试验组患者治疗后临床总有效率为94.00%,高于对照组的78.00%;与治疗前比,治疗后两组患者VAS评分均降低,且试验组低于对照组;JOA评分均升高,且试验组高于对照组;血浆5-HT、SP、TXB2和T/K比值均降低,且试验组低于对照组;血浆6-K-PGF1α均升高,且试验组高于对照组(均P<0.05)。结论温针灸联合牵引治疗腰椎间盘突出所致坐骨神经痛可调控血浆TXB2、6-K-PGF1α水平,促进血液循环,且具有较好的镇痛作用,临床疗效显著提高。Objective To study the effects of warm acupuncture combined with traction on pain substances and TXB2,6-K-PGF1αin treating sciatica caused by lumbar disc herniation.Methods From January 2018 to March 2019,100 patients with sciatica caused by lumbar disc herniation in Yuechi Hospital of Traditional Chinese Medicine were selected and randomly divided into the control group and the experimental group,50 cases in each group.Patients in the control group were received conventional traction therapy,while patients in the experimental group were received warm acupuncture combined with traction therapy.Patients in both groups were treated for 30 days.The clinical efficacy,VAS,JOA scores,plasma 5-HT,SP,TXB2,6-K-PGF1αlevels and T/K ratio were compared between the two groups.Results The clinical total effective rate of patients in the experimental group was 94.00%,which was higher than that of the control group 78.00%;Compared with before treatment,the VAS score of the two groups of patients decreased,and the experimental group was lower than the control group;the JOA score increased after treatment,and the experimental group was higher than the control group;The plasma 5-HT,SP,TXB2 levels and T/K ratio decreased,and the experimental group was lower than the control group;The plasma 6-K-PGF1αincreased,and the experimental group was higher than the control group(all P<0.05).Conclusion Warm acupuncture combined with traction in the treatment of sciatica caused by lumbar disc herniation can regulate the plasma TXB2,6-K-PGF1αlevels,promote blood circulation,and has a better analgesic effect,and the cinical effect enhance significantly.
