摘  要:目的:观察文体活动结合个体化健康教育对结肠造口患者自尊、自我管理能力的影响。方法:选取本院2018年1月-2019年2月收治的108例行结肠造口术的患者,根据非随机临床同期对照研究及结肠造口患者自愿原则,将其分为观察组和对照组。对照组结肠造口患者实施常规干预,观察组结肠造口患者在对照组的基础上开展文体活动并进行个体化健康教育。由8名资深护师根据结肠造口患者的个体化差异(身体状况、年龄、心态、受教育程度等),制定更具针对性的个体化干预方案,干预时间为12个周,以状态自尊量表(SSES)得分为标准,比较两组患者干预前后的自尊情况;采用自我效能评分表,比较两组患者干预前后自我效能得分,评估两组患者的自我管理能力情况;根据生活质量评分表(ADL)得分情况,比较两组结肠造口患者干预前后生活质量状况;采用赫氏希望量表(HHI)对两组患者的希望水平进行评分。结果:干预后观察组患者SSES评分中外表自尊、社会自尊、行为自尊得分显著高于对照组(t=9.904,18.612,9.303;P<0.05);观察组患者干预后行动能力、决策能力、心理能力与应对能力得分显著高于对照组(t=4.956,11.325,6.591,2.598;P<0.05);观察组患者干预后生活情况、社会情况、交流情况、饮食情况与心理情况得分显著高于对照组(t=5.828,5.196,6.989,7.173,5.387;P<0.05);干预后观察组患者对现实和未来的积极态度、采取积极的行动、与他人保持亲密关系得分显著高于对照组(t=4.822,2.701,4.525;P<0.05)。结论:文体活动结合个体化健康教育能有效改善结肠造口患者的自卑情绪,提高患者自尊水平和希望水平,增强患者自我管理能力,改善结肠造口患者生活质量。Objective:To observe effects of recreational activities combined with individualized health education on self-esteem and self-management ability of colostomy patients.Methods:A total of 108 patients who underwent colostomy and were admitted to the hospital from January 2018 to February 2019 were enrolled.According to non-randomized clinical homochronous controlled study and voluntary principle,they were divided into observation group and control group.The control group was given routine intervention,while observation group was given recreational activities combined with individualized health education on basis of control group.According to individualized differences(physical status,age,mentality,education level)of colostomy patients,more targeted individualized intervention plans were developed by eight senior nurses.The intervention course was 12 weeks.Taking scores of state self-esteem scale(SSES)as standard,self-esteem status before and after intervention was compared between two groups.The self-efficacy scales were made on their own.The scores of self-made self-efficacy scales before and after intervention were compared between two groups.The self-management ability of two groups was evaluated.The quality of life before and after intervention was compared between two groups by Activity of Daily Living Scale(ADL).The hope level was scored using the Herth hope index(HHI).Results:Before intervention,there was no significant difference in SSES score,ADL score or self-management ability between the two groups(P>0.05).After intervention,scores of appearance self-esteem,social self-esteem and behavioral self-esteem in SSES in observation group were significantly higher than those in control group and before intervention(t=9.904,18.612,9.303;P<0.05).After intervention,scores of action ability,decision-making ability,mental ability and coping ability in observation group were significantly higher than those in control group and before intervention(t=4.956,11.325,6.591,2.598;P<0.05).After intervention,scores of life,so
