加味当归补血颗粒治疗肿瘤相关性贫血的临床观察

摘　　要：目的:观察加味当归补血颗粒治疗肿瘤相关性贫血(CRA)的疗效及对患者细胞免疫功能的影响。方法:选取2018年1月至2019年12月泸州医学院附属中医院肿瘤科收治的肿瘤相关性贫血患者112例作为研究对象,随机分为对照组和观察组,每组56例。对照组静脉滴注蔗糖铁,皮下注射重组人促红细胞生成素(rhEPO);观察组在对照组治疗的基础上加用当归补血颗粒,8周为1个疗程。疗程结束后进行实验室检查,采用流氏细胞仪检测各淋巴细胞亚群比例,比较临床疗效,记录不良反应。结果:治疗后,观察组有效率为89.2%,高于对照组的66.2%(P<0.05)。与对照组比较,观察组实验室指标Hb、RBC显著增高,RET明显较低;细胞免疫功能指标CD3^+、CD4^+、CD4^+/CD8^+、CD3^+CD16^+CD56^+、CD3^+CD16^+CD56^+显著增高(P<0.05)。观察组不良反应反生率为7.7%,低于对照组的23.1%(P<0.05)。结论:当归补血颗粒联合蔗糖铁注射液、rhEPO治疗CRA安全有效,可能与其增强患者细胞免疫功能有关。Objective:To observe the clinical effects of Modified Danggui Buxue Granules and the its effects on the treatment on the patients of cancer related anemia(CRA)and the cellular immune function.Methods:A total of 100 patients admitted to Oncology Department of the Affiliated Hospital of Traditional Chinese Medicine of Luzhou Medical College were randomly divided into a control group(65 cases)and a treatment group(65 cases).The control group was treated with intravenous infusion of iron sucrose and subcutaneous injection of recombinant human erythropoietin(rhEPO);and the treatment group was treated with Danggui Buxue Granules for 8 weeks as a course of treatment.Laboratory tests,flow cytometry to detect the proportion of lymphocyte subgroups were performed.Clinical efficacy,and adverse reactions were recorded..Results:After treatment,the effective rate of the observation group was 89.2%,which was higher than in the control group of 66.2%(P<0.05).Compared with the control group,laboratory test indexes such as Hb and RBC increased,RET decreased;cellular immune function indexes such as CD3^+,CD4^+,CD4^+/CD8^+,CD3^+CD16^+CD56^+,CD3^+CD16^+CD56^+significantly increased(P<0.05).The occurrence rate of adverse reactions in observation group was 7.7%,which was lower than in the control group of 23.1%(P<0.05).Conclusion:Danggui Buxue Granules combined with iron sucrose injection and rhEPO is effective and safe for treating CRA,which may be related to inhancing the cellular immune function.
