摘　　要：缺血性卒中急性期血压升高与不良预后密切相关,但卒中急性期降压治疗并不能显著改善临床预后。血压通过改变脑血流代谢影响临床预后,但血压对脑血流代谢的影响在不同卒中亚型人群中又存在差异。该文围绕急性缺血性卒中血压与临床预后的关系,系统综述血压变化对不同卒中亚型人群脑大动脉血流、组织灌注及侧支循环的影响,深入探索血压变化与临床预后的病理生理机制,以期增加对卒中早期血压与临床预后关系的理解,为卒中早期个体化血压管理提供参考。The elevated blood pressure in acute period of ischemic stroke is closely associated with poor prognosis.However,some studies found that the management of lowering blood pressure could not improve clinical prognosis.Blood pressure can affect clinical prognosis by affecting cerebral blood flow and metabolism,while its effects are different in people of various stroke subtypes.By reviewing studies about the relationship between blood pressure in acute ischemic stroke and the clinical prognosis,the paper generalized the influences of blood pressure on cerebral blood flow,perfusion,and collaterals circulation in various subtypes of ischemic stroke,tried to explore the pathophysiologic mechanism and provide reference for the individualized blood pressure management strategy in the early stage of stroke.
