摘　　要：目的探讨Derlin-1在膀胱癌组织中的表达及意义,评估其与膀胱癌患者临床特征的相关性及对患者预后的影响。方法回顾性分析成都医学院第一附属医院2013年1月至2015年1月泌尿外科收治的100例膀胱癌患者的临床资料,比较膀胱癌组织和癌旁组织Derlin-1表达情况,依据膀胱癌组织Derlin-1检测结果是否为阳性将其分为阳性组和阴性组,比较两组患者临床特征及其与Derlin-1表达的相关性;以患者死亡或截止日期为随访终点,依据患者是否死亡将其分为死亡组和存活组,采用多因素Logistic回归分析探讨影响膀胱癌患者预后的危险因素。结果Derlin-1在膀胱癌组织中的阳性表达率(68.00%)显著高于癌旁组织(49.00%)(χ^2=12.235,P<0.001);浅表性膀胱癌患者的癌组织Derlin-1阳性表达率显著低于浸润性膀胱癌(P<0.05);阳性组患者临床分期T2~T4期、病理分级G2~G3级、浸润性膀胱癌占比均显著高于阴性组(均P<0.05)。Derlin-1阳性表达与临床分期和病理分级均呈正相关(r=0.538,0.612,均P<0.01)。患者3年存活率为69.47%,多因素Logistic回归分析结果显示,临床分期T2~T4期、淋巴结转移、Derlin-1阳性表达、病理分级G2~G3级和浸润性膀胱癌均是影响膀胱癌患者预后的危险因素(均P<0.05)。结论Derlin-1在膀胱癌组织中高表达,且与膀胱癌患者临床分期、病理分级、病理类型密切相关,可作为膀胱癌发展、转移的重要预测指标。Objective To explore the expression and significance of Derlin-1 in bladder cancer tissues,and to evaluate its correlation with clinical features of patients with bladder cancer and its influence on prognosis.Method The clinical data of 100 patients with bladder cancer admitted to the First Affiliated Hospital of Chengdu Medical College from January 2013 to January 2015 were retrospectively analyzed.Derlin-1 expression was compared between bladder cancer tissues and adjacent tissues.According to whether Derlin-1 result was positive or not,the patients were divided into positive group and negative group,and the clinical features of the two groups were compared,and the correlation between Derlin-1 and pathological features was analyzed.The death or expiration date of patients was used as the follow-up end point,and according to whether the patient died or not,the patients were divided into death group and survival group,and multivariate Logistic regression analysis was used to investigate the risk factors affecting the prognosis of bladder cancer patients.Result The positive expression rate of Derlin-1 in bladder cancer tissues(68.00%)was significantly higher than that in adjacent tissues(49.00%)(χ^2=12.235,P<0.001).The positive Derlin-1 expression rate in superficial bladder cancer was significantly lower than that in invasive bladder cancer(P<0.05).The proportion of clinical stage T2~T4,pathological grade G2~G3 and invasive bladder cancer in positive group were significantly higher than those in negative group(all P<0.05).The positive expression of Derlin-1 were positively correlated with clinical stage and pathological grade(r=0.538,0.612,all P<0.01).The 3-year survival rate in all patients was 69.47%,and multivariate Logistic regression analysis showed that clinical stage T2~T4,lymph node metastasis,Derlin-1 positive expression,pathological grade G2~G3 and invasive bladder cancer were risk factors affecting the prognosis of patients with bladder cancer(all P<0.01).Conclusion Derlin-1 is highly expressed in bla
