摘  要:目的研究首发精神分裂症患者血清中神经调节蛋白-1(NRG-1)水平、脑电图γ活动及认知功能的相关性。方法随机选取2018年1月—2019年8月于山东省青岛市精神卫生中心(以下简称“我院”)就诊的首发精神分裂症患者80例作为研究组,另选取同期我院健康体检人群50名作为对照组。检测两组血清NRG-1水平及脑电图γ活动情况,使用阳性和阴性症状量表(PANSS)评估研究组病情,采用MATRICS公认认知成套测验(MCCB)评估两组认知功能,分析研究组NRG-1水平、脑电图γ活动及认知功能的关系。结果研究组血清NRG-1水平和脑电图γ活动阳性比例高于对照组,而MCCB中的处理速度、注意警觉、工作记忆、语言记忆、视觉记忆及推理解决得分均低于对照组,差异均有统计学意义(均P<0.05)。与脑电图γ活动阴性患者比较,脑电图γ活动阳性患者NRG-1水平及MCCB各因子得分(处理速度、注意警觉、工作记忆、语言记忆、视觉记忆及推理解决)均降低,而PANSS阳性症状、阴性症状、一般精神病理及总分均升高,差异均有统计学意义(均P<0.05)。研究组NRG-1水平与语言记忆及推理解决得分呈正相关(r=0.433、0.554,P<0.01),与PANSS总分呈负相关(r=-0.584,P<0.01)。研究组脑电图γ活动与NRG-1水平、语言记忆及推理解决得分呈负相关(r=-0.520、-0.610、-0.541,P<0.01),与PANSS总分呈正相关(r=0.687,P<0.01)。结论首发精神分裂症患者血清NRG-1水平明显降低,脑电图γ活动明显增多,且二者与患者病情及认知功能均有明显的相关性,临床可据此给予患者早期个性化治疗。Objective To investigate the correlation among serum neuregulin-1(NRG-1)level,EEGγactivity and cognitive function in patients with first-episode schizophrenia.Methods A total of 80 first-episode schizophrenic patients admitted to Qingdao Mental Health Center,Shandong Province(hereinafter referred to as“our hospital”)from January 2018 to August 2019 were randomly selected as the study group,and another 50 healthy people who underwent physical examination in our hospital during the same period were selected as the control group.The serum NRG-1 level and EEGγactivity of the two groups were detected.The condition of the study group was evaluated by the positive and negative symptom scale(PANSS),and the cognitive function of the two groups was evaluated by the MATRICS recognized cognitive battery test(MCCB).The correlation among NRG-1 level,EEGγactivity and cognitive function in the study group was analyzed.Results The NRG-1 level and the positive proportion of EEGγactivity in the study group were higher than those in the control group,while the processing speed,attention alertness,working memory,verbal memory,visual memory and reasoning resolution scores in the MCCB were all lower than those in the control group,and the differences were statistically significant(all P<0.05).Compared with patients with EEGγactivity in negative,the NRG-1 level and each MCCB factor scores(processing speed,attention alertness,working memory,verbal memory,visual memory and reasoning resolution)of patients with positive EEGγactivity were all decreased,and PANSS spirit of positive symptoms,negative symptoms,general psychopathology and total score were all increased,and the differences were statistically significant(all P<0.05).The level of NRG-1 in the study group was positively correlated with the score of verbal memory and reasoning resolution(r=0.433,0.554;P<0.01),but negatively correlated with the total score of PANSS(r=-0.584,P<0.01).The EEGγactivity in the study group was negatively correlated with NRG-1 level,verbal memory
