甲状腺髓样癌中降钙素和降钙素原的表达及临床意义

摘　　要：目的探讨降钙素(CTn)和降钙素原(PCT)在散发型甲状腺髓样癌(SMTC)组织中的表达特征。方法采用免疫组化法检测SMTC(64例)、结节性甲状腺肿(55例)和正常甲状腺(35例)组织中CTn和PCT的表达水平。结果SMTC组织中CTn、PCT的表达率分别为90.6%(58/64)、67.2%(43/64),明显高于结节性甲状腺肿组织的78.2%(43/55)、47.3%(26/55)和正常甲状腺组织的68.6%(24/35)、37.1%(13/35),差异有统计学意义(P<0.05)。CTn、PCT表达与患者性别、年龄和包膜侵犯均无关(P>0.05),而与肿瘤大小和淋巴结转移有关(P<0.05)。SMTC组织中CTn、PCT表达水平具有显著相关关系(r=0.269,P=0.041),同时CTn阳性表达强度与RET原癌基因突变呈正相关关系(r=0.507,P<0.001),而PCT表达与RET突变无关(r=0.188,P=0.136)。结论CTn、PCT的高表达与甲状腺髓样癌的病情进展有关,PCT可作为甲状腺髓样癌诊断指标并对评估预后有一定价值。Objective To investigate the expression characteristics of calcitonin(CTn)and procalcitonin(PCT)in the tissues of sporadic medullary thyroid carcinoma(SMTC).Methods Immunohistochemical method was used to detect the expression of CTn and PCT in SMTC(64 cases),nodular goiter(55 cases)and normal thyroid tissue(35 cases).Results The expression rates of CTn and PCT in SMTC tissues were 90.6%(58/64)and 67.2%(43/64),respectively,significantly higher than that in nodular thyroid tissues 78.2%(43/55),47.3%(26/55)and normal thyroid tissues 68.6%(24/35)and 37.1%(13/35),with a statistically significant difference(P<0.05).CTn and PCT expression were not related to gender,age and capsule invasion of SMTC patients(P>0.05),but to tumor size and lymph node metastasis(P<0.05).CTn and PCT expression levels were significantly correlated in SMTC tissues(r=0.269,P=0.041),and positive expression intensity of CTn was positively correlated with RET mutations(r=0.507,P<0.001),while PCT expression was not correlated with RET mutations(r=0.188,P=0.136).Conclusion The high expression of CTn and PCT is related to the disease progression of medullary carcinoma,PCT can be used as a diagnostic index of medullary thyroid and has certain value in evaluating prognosis.
