牙列缺损的修复方式对口腔健康相关生活质量的影响  

The Influence of Prosthodontic Rehabilitation Modalities on Oral Health-Related Quality of Life in Partially Edentulous Patients

摘　　要：目的分析牙列缺损患者可摘局部义齿(removable dental prostheses,RDPs)、固定义齿(fixed dental prostheses,FDPs)及种植固定义齿(implant-supported fixed prostheses,ISFPs)修复后口腔健康相关生活质量(oral healthrelated quality of life,OHRQoL)的变化。方法运用口腔健康程度量表(oral health impact profile,OHIP-14)记录牙列缺损患者修复前和修复后6周的结果。结果修复前生理性疼痛OHIP-14分数最高,其次是心理不适,三组治疗后OHIP-14分数降低。协方差分析显示,排除患者修复前的口腔状况对修复后效果的影响后,不同修复方式修复后患者的OHRQoL不同,RDPs组高于FDPs组及ISFPs组(P均>0.05)。结论牙齿缺失后对OHRQoL的影响主要表现在生理性疼痛和心理不适两个领域。FDPs及ISFPs修复优于RDPs。Objective To analyze the changes of oral health-related quality of life(OHRQoL)in patients with dentition defect after removable dentures(RDPS),fixed dentures(FDPs)and implant supported fixed dentures(ISFPs).Methods OHRQoL was measured using the Oral Health Impact Profile-Chinese version(OHIP-14)before treatment and 6 weeks after treatment.Results Before treatment,the scores of domain 2(physical pain)were the highest,and the second were domain 3(psychological discomfort).After treatment,the OHRQoL score of three groups were decreased.Analysis of covariance showed that exclude the influence of the patients'oral condition before the restoration on the effect after the restoration,the scores of the patients after different restoration methods were different,RDPs were higher than FDPs and ISFPs(P all>0.05).Conclusion Before treatment,the effect of tooth loss on OHRQoL was mainly reflected in two aspects:physical pain and psychological discomfort domain.The OHRQoL of the patients was all significantly improved after treatment with FDPs,ISFPs and RDPs,and the FDPs,ISFPs were superior than RDPs.
