经皮单侧入路与双侧乳晕入路在甲状腺切除术中的效果观察

摘　　要：目的分析经皮单侧入路与双侧乳晕入路在甲状腺切除术中的效果。方法选择130例甲状腺良性肿瘤患者为研究对象,将其分为2组,其中对照组65例患者实施经皮双侧乳晕入路甲状腺切除术,研究组65例患者实施经皮单侧乳晕入路甲状腺切除术。比较2组患者手术时间、皮下解剖面积、失血量、围手术期并发症、术后疼痛、住院时间及满意度。结果研究组患者的平均手术时间高于对照组,差异有统计学意义(P<0.05)。研究组患者平均皮下解剖面积及术后引流量明显低于对照组,而非常满意度比例高于对照组(P<0.05)。结论经皮单侧乳晕入路甲状腺切除术安全、可行,与双侧乳晕入路甲状腺切除术方法相比,具有微创、美容效果好的优点。经皮单侧乳晕入路甲状腺切除术在技术上更具挑战性,可以作为对治疗甲状腺良性肿瘤患者的有效手段。Objective To analyze the effect of percutaneous unilateral approach and bilateral areola approach in thyroidectomy.Methods 130 patients with thyroid benign tumor who were hospitalized were randomly divided into two groups,65 patients in the control group were treated with percutaneous bilateral areola thyroidectomy,and 65 patients in the study group were treated with percutaneous unilateral areola thyroidectomy.The operation time,subcutaneous anatomic area,blood loss,perioperative complications,postoperative pain,hospitalization time and satisfaction were observed and compared between the two groups.Results The average operation time of the study group was higher than that of the control group,the difference was statistically significant(P<0.05).In addition,the average subcutaneous anatomic area and postoperative drainage volume of patients in the study group were significantly lower than those in the control group,while the very satisfied proportion was higher than that in the control group(P<0.05).Conclusion Thyroidectomy via unilateral areola approach is safe and feasible.Compared with bilateral areola approach,it has the advantages of minimally invasive and good cosmetic effect.Thyroidectomy via unilateral areola approach is technically more challenging and can be used as an effective method for the treatment of thyroid benign tumors.
