摘  要:目的探讨青少年抑郁症患者脑内神经递质水平变化与病情严重程度的相关性。方法选择2017年2月至2018年5月在烟台业达医院进行治疗的36例青少年抑郁症患者作为观察对象,依据抑郁症严重程度分为轻度抑郁症组[抑郁自评量表(SDS)评分53~62分,14例]、中度抑郁症组(SDS评分63~72分,12例)及重度抑郁症组(SDS评分>73分,10例),另选取同期接受健康体检的34例健康志愿者作为对照组,采用中枢神经递质分析系统与定量数字脑电图仪对四组研究对象的去甲肾上腺素(NE)、兴奋性神经递质(EAA)、抑制性神经递质(INH)、多巴胺(DA)、5-羟色胺(5-HT)、乙酰胆碱(ACH)进行检测,分析青少年抑郁症病情严重程度与脑内神经递质变化的相关性。结果轻度抑郁症组SDS评分为(57.42±2.16)分,中度抑郁症组SDS评分为(68.74±2.20)分,重度抑郁症组SDS评分为(87.49±3.51)分。对照组NE、INH水平与轻度抑郁症组、中度抑郁症组及重度抑郁症组比较,差异未见统计学意义(P>0.05);对照组DA、ACH、EAA水平高于轻度抑郁症组、中度抑郁症组及重度抑郁症组,且5-HT低于轻度抑郁症组、中度抑郁症组及重度抑郁症组,差异有统计学意义(P<0.05)。经Pearson相关性分析发现,青少年SDS评分与DA、ACH、EAA及5-HT水平呈负相关性(P<0.05);青少年SDS评分与NE、INE水平无相关性(P>0.05)。结论青少年抑郁症严重程度与脑内神经递质水平变化之间具有相关性,且呈负相关关系,对青少年抑郁症患者可通过对脑内神经递质水平变化进行观察,以达到尽早发现与尽早治疗。Objective To investigate the relationship between the change of neurotransmitter level in the brain and the severity of adolescent depression.Methods A total of 36 adolescent depression patients who were treated in Yantai Yeda Hospital from February 2017 to May 2018 were selected as the research objects. According to the severity of depression, they were divided into mild depression group (14 cases), in which Self-rating Depression Scale (SDS) score was 53-62, moderate depression group (SDS score 63-72, 12 cases) and major depression group (SDS score>73, 10 cases). And 34 cases of healthy persons who received physical examination in the same period were selected as the control group. Noradrenaline (NE), excitatory amino acids (EAA), enhibitory neurotransmitter (INH), dopamine (DA), 5-hydroxytryptamine (5-HT) and acetylcholine (ACH) of each group were detected by central neurotransmitter analysis system and quantitative digital electroencephalograph. The correlation between the severity of depression and neurotransmitter changes in brain was analyzed.Results SDS score was 57.42±2.16 in mild depression group, 68.74±2.20 in moderate depression group, and 87.49±3.51 in major depression group. There was no significant difference in NE or INH level between the control group and mild depression group, between moderate depression group and major depression group (P>0.05). DA, ACH and EAA levels in the control group were higher than those in the mild depression group, moderate depression group and major depression group, while 5-HT of the control group was lower than those of the mild depression group, moderate depression group and major depression group, with statistically significant differences (P<0.05). Pearson correlation analysis showed that SDS score was negatively correlated with levels DA, ACH, EAA and 5-HT in adolescents (P<0.05).
