摘 要:目的探究人乳头瘤病毒(Human papilloma virus,HPV)16/18型感染宫颈癌患者外周血及癌组织中程序性死亡受体/配体(Programmed death receptor-1/ligand-1,PD-1/L1)水平与临床特征相关性。方法选择西北妇女儿童医院妇科2017年8月-2019年8月确诊为高危型HPV16/18感染的宫颈癌患者82例作为宫颈癌组,另选择HPV16/18阳性的宫颈上皮内瘤样变(Cervical intraepithelial neoplasia,CIN)患者42例作为CIN组及健康体检者40例作为健康对照组。分别采用免疫组化法检测其癌组织中PD-1、PD-L1蛋白表达水平并采用qRT-PCR法检测其外周血PD-1、PD-L1mRNA表达水平,对不同年龄、FIGO分期、肿瘤大小、分化程度、瘤旁浸润及淋巴结转移情况PD-1、PD-L1水平进行比较。结果宫颈癌组PD-1、PD-L1蛋白表达阳性率高于CIN组与健康对照组,CIN组高于健康对照组,差异具有统计学意义(P<0.05);宫颈癌组外周血PD-1、PD-L1 mRNA水平高于CIN组及健康对照组,CIN组PD-1、PD-L1水平高于健康对照组(P<0.05);癌组织PD-1、PD-L1蛋白表达在不同FIGO分期、不同肿瘤大小、不同分化程度中差异具有统计学意义(P<0.05);外周血PD-1mRNA相对水平在不同FIGO分期、不同肿瘤大小、是否发生淋巴结转移差异具有统计学意义(P<0.05);外周血PD-L1mRNA相对水平在不同FIGO分期、不同肿瘤分化程度、是否发生宫旁浸润差异具有统计学意义(P<0.05)。结论 PD-1/PD-L1通路参与了宫颈癌的发展过程,可作为宫颈癌临床治疗的免疫靶点。OBJECTIVE To explore the correlation between levels of programmed death receptor-1(PD-1) and programmed death receptor ligand-1(PD-L1) in peripheral blood and cancer tissues and clinical characteristics in cervical cancer patients infected by human papilloma viruses(HPV) type 16 and type 18. METHODS Eighty-two patients with cervical cancer infected with high-risk HPV16 and HPV18 viruses from the Department of Gynecology in our hospital during Aug. 2017 to Aug. 2019 were enrolled as the cervical cancer group;42 patients with HPV16/18-positive cervical intraepithelial neoplasia(CIN) were in the CIN group;40 healthy volunteers under physical examination during the same period were in the healthy control group. Immunohistochemical method was used to detect the expression levels of PD-1 and PD-L1 protein in cancer tissue and qRT-PCR method was used to detect the mRNA expression levels of PD-1 and PD-L1 in peripheral blood. PD-1 and PD-L1 levels in patients with different ages, FIGO staging, tumor sizes, tumor differentiation, degree of paraneoplastic invasion and lymph node metastasis were compared. RESULTS The positive expression rates of PD-1 and PD-L1 protein in the cervical cancer group were significantly higher than that in the CIN group and the healthy control group;the positive expression rates of PD-1 and PD-L1 protein in the CIN group was significantly higher than that in the healthy control group(P<0.05);MRNA levels of PD-1 and PD-L1 in peripheral blood of the cervical cancer group were significantly higher than that in the CIN group and healthy control group;PD-1 and PD-L1 levels in the CIN group were significantly higher than that in the healthy control group(P<0.05);PD-1 and PD-L1 protein expression in cancer tissues in different FIGO stages, different tumor sizes and degree of differentiation were significant(P<0.05). The relative mRNA expression levels of PD-1 in peripheral blood was different in patients with different FIGO stages, different tumor sizes, and whether lymph node metastasis occurred(P<0.0
