食管胃交界处癌根治性切除术的预后危险因素分析

摘　　要：目的:分析食管胃交界处癌根治性切除术的预后危险因素。方法:抽取2016年11月至2018年11月滕州市中心人民医院收治的食管胃交界处癌患者120例,均行食管胃交界处癌根治性切除术,分析TNM不同分期患者的术后并发症、生活质量、生存情况,应用Logistic回归分析术后复发的影响因素。结果:120例中TNMⅠ~Ⅳ期患者术后并发症、生活质量、6个月~5年的生存率比较,差异有统计学意义(P<0.05)。术后复发的独立影响因素为性别、年龄、病灶长径、是否接受化疗、复发间隔时间、住院时间、手术时间、TNM分期(OR=2.013、1.917、1.968、2.042、1.986、1.785、1.635、1.422,P<0.05)。结论:食管胃交界处癌行根治性切除术后复发的危险因素与患者自身、病灶、治疗情况相关,制定特征性个体防护方案,有可能改善预后。Objective To analyze the prognostic risk factors of radical resection of esophagogastric junction cancer.Methods A total of 120 patients with esophagogastric junction cancer treated in Tengzhou Central People’s Hospital from November 2016 to November 2018 were selected.All patients underwent radical resection of the esophagogastric junction cancer.The postoperative complications,quality of life,and survival of patients with different stages of TNM were analyzed,and Logistic regression was used to analyze the influencing factors of postoperative recurrence.Results There were significant differences in the postoperative complications,quality of life,and the survival rate of 6 months to 5 years in 120 patients with TNM stage I to IV(P<0.05).The independent influencing factors of postoperative recurrence were gender,age,focal length,whether to accept chemotherapy,recurrence interval,length of hospital stay,operation time,TNM stage(OR=2.013,1.917,1.968,2.042,1.986,1.785,1.635,1.422,P<0.05).Conclusions The risk factors of recurrence after radical resection of esophagogastric junction cancer are related to the patient,lesions,and treatment.It is possible to develop a characteristic individual protection plan to improve prognosis.
