基于START流程紧急培训检伤分类护士的可行性及相关因素分析

摘　　要：目的探讨通过快速紧急专项培训培养灾害检伤分类护士的可行性,并对相关影响因素进行分析。方法基于START检伤分类方案实施30 min的专项培训,在培训前后进行情景模拟测试。测量指标包括:检伤分类完成时间、检伤分类正确率、检伤过度与检伤不足率、漏检率,评估被研究者的检伤分类能力变化。结果灾害检伤分类快速紧急培训前后检伤分类用时缩短,检伤正确率提高,检伤过度、检伤不足及漏检率均降低,差异有统计学意义(P<0.05),护士检伤分类能力提高;不同学历、职称、护龄、急诊工龄护士在检伤用时、检伤正确率、检伤过度、检伤不足及漏检方面的变化无统计学差异(P>0.05);有无灾害救援经历在检伤用时方面的变化差异有统计学意义(P<0.05),在检伤正确率、检伤过度、检伤不足及漏检率方面的培训变化差异无统计学意义(P>0.05)。结论通过快速紧急的专项培训,短时间培训出具备相应能力的检伤分类护士应对灾害救援检伤分类的需要,因实施简单,遴选人员限制少,受训护士能力变化明显,具有可行性和可操作性。Objectives To discuss the feasibility and analyze the related factors of short-term emergency triage training in disaster relief.Methods A 30 min triage training according to START triage algorithm was developed.Simulation tests were hold to examine the nurses’triage skills before and after the training.The change of triage time,triage accuracy,over-triage and under-triage rate,and the number of patients without triage were recorded.Results The nurses’triage skill in disaster relief was significantly improved after training.After training,the nurses could triage more quickly.The triage accuracy was improved.Over-triage and under-triage rate,and the number of patients without triage were decreased(P<0.05).There was no significant difference of the triage time,triage accuracy,over-triage and under-triage rate between different groups with or without triage training(P>0.05).But the triage time presented significant difference in different previous disaster relief experience groups(P<0.05).Conclusion It is easy to carry out the short-term emergency triage training.And there is no special limited on nurses selection.It is feasible to improve the nurses’triage skills in disaster relief through short-term emergency triage training.
