D-二聚体、CEA及CA19-9联合检测用于胰腺癌诊断的敏感性与特异性探讨

摘　　要：目的联合检测D-二聚体(D-D)、癌胚抗原(CEA)及糖抗原19-9(CA19-9),分析其诊断胰腺癌的敏感性与特异性。方法选取200例胰腺癌患者、100例健康体检者及100例良性病变者作为研究对象,检测血中D-D、CEA及CA19-9水平,并评估各指标的诊断价值。结果观察组、良性病变对照组患者血清中D-D、CEA及CA19-9水平均明显高于对照组(P<0.05);D-D、CEA及CA19-9三指标联合检测诊断胰腺癌的AUC为0.931,显著高于单独检测某指标或任意两指标联合检测。结论通过联合检测血中D-D、CEA及CA19-9水平,可有效提高早期筛查胰腺癌的灵敏度及特异度,具有较高的临床应用价值。Objective To study the combination detection on D-Dimer(D-D),carcinoembryonic antigen(CEA)and carbohydrate antigen 19-9(CA19-9),and analyze their sensitivity and specificity in the diagnosis of pancreatic cancer.Methods The levels of D-D,CEA and CA19-9 in blood were measured in 200 patients with pancreatic cancer,100 healthy people and 100 benign lesions,the diagnostic value in each index was evaluated.Results The average of D-D,CEA and CA19-9 level in the observation group were significantly higher than those in the control group(P<0.05),the AUC combined detection of D-D,CEA and CA19-9 in the diagnosis of pancreatic cancer was 0.931,which is significantly higher than those in the single detection index or any combination of two indexes.Conclusion The combined detection of D-D,CEA and CA19-9 in blood can effectively improve the sensitivity and specificity in early screening pancreatic cancer,which has a high clinical application value.
