摘 要:目的探讨X线、CT、MRI检查及不同组合方案诊断原发性脊柱骨肿瘤的价值。方法回顾性分析128例原发性脊柱骨肿瘤患者的影像学资料,所有患者均接受X线、CT和MRI检查。以病理学检查结果为金标准,比较X线、CT、MRI检查以及不同影像学检查组合方案(X线+CT、X线+MRI、X线+CT+MRI)的灵敏度、特异度、准确度、阳性预测值、阴性预测值。结果128例原发性脊柱骨肿瘤患者中,病理检查提示恶性骨肿瘤42例,良性骨肿瘤86例。采用单一方法诊断时,CT检查的灵敏度、阴性预测值最高,MRI检查的特异度、准确度、阳性预测值最高。采用联合诊断时,X线+CT+MRI的灵敏度、特异度、准确度、阳性预测值、阴性预测值均最高。结论在单一检查方法中,CT和MRI诊断原发性脊柱骨肿瘤的准确性、敏感性、特异性均高于X线;在联合检查方案中,X线+CT+MRI方案的诊断效能最好。Objective To explore the value of plain film radiography,CT,MRI,and different combination schemes in the diagnosis of primary spinal bone tumors.Methods The imaging data of 128 patients with primary spinal bone tumors were analyzed retrospectively.All patients underwent plain film radiography,CT,and MRI examinations.Using the pathological examination results as the gold standard,the sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of plain film radiography,CT,MRI examination,and different imaging examination combination schemes(plain film radiography+CT,plain film radiography+MRI,plain film radiography+CT+MRI)were compared.Results Among the 128 patients with primary spinal bone tumors,42 were malignant bone tumors and 86 were benign bone tumors revealed by pathological examination.In the case of diagnosis by single examination,CT examination achieved the highest sensitivity and negative predictive value,while MRI examination obtained the highest specificity,accuracy and positive predictive value.In the case of diagnosis by combination examination,the combination of plain film radiography,CT and MRI got the highest sensitivity,specificity,accuracy,positive predictive value and negative predictive value.Conclusion In terms of single examination method,CT/MRI achieves higher accuracy,sensitivity,and specificity than plain film radiography;in terms of combined examination schemes,plain film radiography+CT+MRI scheme exhibits the best diagnostic efficiency.
