18F-FDG PET-CT评价NSCLC患者早期化疗的价值

摘　　要：目的探究18氟-脱氧葡萄糖(18 F-FDG)正电子发射计算机断层扫描(PET-CT)在评价非小细胞肺癌(NSCLC)患者早期化疗效果中的应用价值。方法选择72例于我院接受化疗的NSCLC患者,于化疗前及化疗1周期后对患者进行18 F-FDG PET-CT扫描,化疗2个周期后进行常规CT扫描,分析图像,以RECIST标准为金标准,分析18 F-FDG PET-CT在NSCLC患者早期化疗的评估价值。结果18 F-FDG PET-CT扫描SUVmax值变化幅度评价疗效:化疗1周期后,72例患者中41例患者代谢缓解,31例患者代谢无缓解,代谢缓解率56.94%(41/72);RECIST标准评价疗效:化疗2周期后,72例患者中完全缓解、部分缓解、疾病稳定、疾病进展为0例、30例、34例、8例,疾病控制率88.89%(64/72);30例患者为客观缓解,42例患者为客观无效,客观缓解率41.67%(30/72);RECIST标准评价与1818F-FDG PET-CT评价的缓解率比较差异无统计学意义(χ^2=3.362,P>0.05),并具有较高一致性(Kappa值=0.430,P<0.05),18 F-FDG PET-CT扫描评价NSCLC患者早期化疗疗效的灵敏度、特异度、准确率为83.33%和61.90%和70.83%。结论18 F-FDG PET-CT扫描于NSCLC患者早期化疗即可较好评价患者疗效,有助于指导临床调整患者化疗方案。Objective To explore the application value of 18fluorodeoxyglucose(18 F-FDG)positron emission computed tomography(PET-CT)in evaluating the effect of early chemotherapy in patients with non-small cell lung cancer(NSCLC).Methods 72 patients with NSCLC who received chemotherapy in our hospital were selected and underwent 18 F-FDG PET-CT scan before and after one cycle of chemotherapy.Results The change range of SUVmax value of 18 F-FDG PET-CT scan was used to evaluate the curative effect.After one cycle of chemotherapy,41 patients had metabolic remission,31 patients had no metabolic remission,the metabolic remission rate was 56.94%(41/72).The curative effect was evaluated by RECIST standard.After two cycles of chemotherapy,the SUVmax value of 18 F-FDG PET-CT was evaluated.Among 72 patients,there were 0,30,34 and 8 patients with complete remission,partial remission,disease stability and disease progression,and the disease control rate was 88.89%(64/72);30 patients were objective remission,42 patients were objective invalid,and the objective remission rate was 41.67%(30/72);There was no significant difference between RECIST standard evaluation and 18 F-FDG PET-CT evaluation in the remission rate(χ^2=3.362,P>0.05),and has high consistency(kappa value=0.430,P<0.05).The sensitivity,specificity and accuracy of 18 F-FDG PET-CT scan in evaluating the efficacy of early chemotherapy in NSCLC patients were 83.33%,61.90% and 70.83%,respectively.Conclusion 18 F-FDG PET-CT scan can better evaluate the curative effect of NSCLC patients with early chemotherapy,which is helpful to guide the clinical adjustment of chemotherapy regimen.
