摘 要:目的探讨P16、HPV L-1壳蛋白的表达及在早期宫颈癌、宫颈上皮内瘤变中的筛查价值。方法本次研究纳入67例CIN患者及15例早期宫颈鳞状上皮细胞癌患者,另选取慢性宫颈炎患者31例,应用第二代杂交捕获法及免疫组化法检测HR-HPV载量、P16及HPV L-1壳蛋白表达并对相关性进行分析。结果P16蛋白在早期宫颈癌、HSIL、LSIL及慢性宫颈炎阳性表达率升高,HPV L-1壳蛋白阳性表达率有显著差异(P<0.05)。随着病毒载量不断增加,HPV L-1及P16蛋白阳性表达率均出现变化(P<0.05)。慢性宫颈炎P16蛋白表达阳性率及高危型HPV病毒呈正相关(P<0.05)。LSIL病变中P16蛋白表达阳性率与高危型HPV病毒呈正相关(P<0.05)。HSIL病变中P16蛋白表达阳性率与高危型HPV病毒无相关性(P>0.05)。结论P16、HPV L-1壳蛋白表达阳性在CIN及早期宫颈癌筛查中的应用价值较高。Objective To explore the expression of P16 and HPV L-1 capsid proteins and their screening value in early cervical cancer and cervical intraepithelial neoplasia.Methods This study included 67 patients with CIN,15 patients with early cervical squamous cell carcinoma,and 31 patients with chronic cervicitis.The HR-HPV load,expression of P16 and HPV L-1 capsid protein were detected by the second generation hybrid capture method and immunohistochemistry.The second-generation hybrid capture method and immunohistochemical method were used to detect HR-HPV load and P16.And their correlation was analyzed.Results The positive expression rate of P16 protein in early cervical cancer,HSIL,LSIL and chronic cervicitis increased.The positive expression rate of HPV L-1 capsid protein was significantly different(P<0.05).As the viral load continued to increase,the positive expression rates of HPV L-1 and P16 proteins all changed(P<0.05).The positive rate of P16 protein expression in chronic cervicitis was positively correlated with high-risk HPV virus(P<0.05).The positive rate of P16 protein expression in LSIL lesions was positively correlated with high-risk HPV virus(P<0.05).The positive rate of P16 protein expression in HSIL lesions was not related to high-risk HPV virus(P>0.05).Conclusion The positive expression of P16 and HPV L-1 capsid protein is of high value in CIN and early cervical cancer screening.
