摘  要:目的探讨血清黏蛋白-1(MUC-1)、胸苷激酶1(TK1)及癌胚抗原(CEA)检测对老年直肠癌病理特征的关系,并分析其对预后的评估价值。方法选取2016年1月~2017年1月海南医学院第二附属医院诊治的120例直肠癌患者为试验组,65例炎性肠病患者为炎性组,50例健康体检者为健康组。检测各组血清MUC-1、TK1及CEA水平,分析血清MUC-1、TK1及CEA表达与直肠癌病理特征和预后的关系。结果试验组患者血清MUC-1、TK1及CEA水平均高于炎性组及健康组,差异有统计学意义(P<0.05);TNM分期为Ⅲ期、中低分化和淋巴结转移患者血清MUC-1、TK1及CEA水平分别高于Ⅱ期、高分化和无淋巴结转移患者,差异有统计学意义(P<0.05)。预后不良患者血清MUC-1、TK1及CEA水平均明显高于预后良好患者,差异有统计学意义(P<0.05)。预测直肠癌患者预后的ROC曲线下面积(AUC)分别为0.855、0.861、0.803,MUC-1、TK1预测直肠癌预后的灵敏度明显高于CEA,差异有统计学意义(P<0.05),而MUC-1、TK1及CEA三项指标预测特异度差异无统计学意义(P>0.05)。MUC-1、TK1与CEA低表达患者3年生存率显著大于高表达患者,MUC-1、TK1与CEA低表达患者生存时间也长于高表达患者,差异有统计学意义(P<0.05)。结论直肠癌患者血清MUC-1、TK1及CEA表达与肿瘤的病理特征有关,检测血清MUC-1、TK1及CEA水平有助于评估直肠癌患者预后情况。Objective To investigate the relationship between serum mucin-1(MUC-1)and thymidine kinase 1(TK1)and carcino-embryonic antigen(CEA)in elderly patients with rectal cancer and to analyze their value in evaluating prognosis.Methods 120 patients with rectal cancer diagnosed and treated in our hospital in January 2016 and January 2017 were selected as experimental group,65 patients with inflammatory bowel disease were in inflammatory group,and 50 healthy subjects were in healthy group.Serum MUC-1,TK1 and CEA levels were detected in each group.The relationship between serum MUC-1,TK1 and CEA expression and pathological features and prognosis of rectal cancer was analyzed.Results The levels of MUC-1,TK1 and CEA in experimental group were higher than those in inflammatory group and healthy group(P<0.05).Serum MUC-1,TK1 and CEA levels in patients with TNM stageⅢor medium-low staged differentiation or lymph node metastasis were higher than those in the patients with TNM stageⅡor high staged differentiation or without lymph node metastasis(P<0.05).The levels of MUC-1,TK1 and CEA in patients with poor prognosis were significantly higher than those in patients with good prognosis(P<0.05).The area under the ROC curve(AUC)for prognosis of rectal cancer predicted by MUC-1,TK1 and CEA was 0.855 and 0.861 and 0.803,respectively.The sensitivity of MUC-1 and TK1 in predicting the prognosis of rectal cancer was significantly higher than that of CEA(P<0.05).However,there was no statistical difference in the prediction specificity of the three indicators(P>0.05).The 3-year survival rate of patients with low expression of MUC-1,TK1 and CEA was significantly higher than that of patients with high expression,and the survival time of patients with low expression of MUC-1,TK1 and CEA was also longer than that of patients with high expression(P<0.05).Conclusion The expressions of serum MUC-1,TK1 and CEA in rectal cancer patients are related to pathological features.Detection of serum MUC-1,TK1 and CEA is helpful to evaluate the prognosis
