摘　　要：目的探讨乳腺癌根治术后放射治疗引发放射性皮炎的临床预防措施、治疗方法和护理要点,以期促进放射性皮炎恢复,减轻患者的痛苦。方法选取我院放疗科收治的650例乳腺癌接受放射性治疗的患者,将其随机分为观察组(n=350,放射性治疗+卤米松软膏)和对照组(n=300,单纯放射性治疗)。对于已经发生放射性皮炎患者,根据临床症状给予相应对症治疗。分析两组患者接受放疗后放射性皮炎的发生情况。结果观察组患者中发生放射性皮炎58例(16.57%),按皮肤损伤程度进行分级,Ⅰ、Ⅱ、Ⅲ、Ⅳ度放射性皮炎患者分别为46、7、5、0例。对照组患者中发生放射性皮炎168例(56.00%),Ⅰ、Ⅱ、Ⅲ、Ⅳ度放射性皮炎患者分别为116、24、26、2例。两组患者的放射性皮炎严重程度比较,差异具有统计学意义(P<0.05)。结论卤米松软膏外用能够有效预防乳腺癌放疗后放射性皮炎的发生,并减轻放射性皮炎严重程度。对于已经发生放射性皮炎患者,可以根据病情采用不同的治疗方案,以减轻皮损、加速愈合,提高患者的生活质量。Objective To explore the clinical preventive measures,treatments and main points of nursing of radiation dermatitis caused by radiotherapy for breast cancer patients received radical mastectomy,in order to promote the recovery of radiation dermatitis and alleviate the suffering of patients.Methods A total of 650 breast cancer patients who received radiation therapy in the radiotherapy department of our hospital were selected and randomly divided into observation group(n=350,radiotherapy+halometasone cream)and control group(n=300,radiotherapy alone).The patients who had suffered radiation dermatitis were given corresponding symptomatic treatment according to their clinical symptoms.The incidence of radiodermatitis after radiotherapy in both groups was analyzed.Results In the observation group,58 cases(16.57%)developed radiation dermatitis,and there were 46,7,5 and 0 cases of radiation dermatitis of degreeⅠ,Ⅱ,ⅢandⅣrespectively according to the degree of skin injury.In the control group,168 cases(56.00%)developed radiation dermatitis,and there were 116,24,26 and 2 cases of radiation dermatitis of degreeⅠ,Ⅱ,ⅢandⅣrespectively.The difference in the severity of radiation dermatitis between the two groups was statistically significant(P<0.05).Conclusion External use of halometasone cream can effectively prevent radiation dermatitis after breast cancer radiotherapy and reduce the severity of radiation dermatitis.For patients with radiation dermatitis,different treatment options can be adopted according to the condition,so as to reduce skin damage,accelerate healing,and improve the quality of life of patients.
