摘　　要：目的探讨自体髂骨植骨联合Y型钛板内固定在胸骨柄肿瘤切除术后胸壁重建中的应用效果。方法回顾性分析陕西省人民医院胸外科收治的自体髂骨植骨联合Y型钛板内固定治疗的5例胸骨柄肿瘤患者的临床资料,术后通过肺部并发症、局部感染、皮瓣坏死、假体松动、反常呼吸等并发症发生情况来评估手术的安全性和有效性。结果本研究5例患者中,行胸骨全切除术患者1例,胸骨次全切术4例。术后评估呼吸运动、胸廓外形和肺功能均正常,1例患者发生肺部炎症,抗炎治疗后治愈,未发生局部感染、皮瓣坏死、假体松动、反常呼吸等并发症。术后复查胸部CT显示胸廓外形良好,髂骨及内固定位置固定。1例患者术后26个月行内固定取出术,术后胸部CT提示髂骨与双侧锁骨及肋骨成骨完整。结论对于胸骨柄肿瘤的患者而言,自体髂骨植骨联合Y型钛板内固定胸骨重建是一种安全可靠的术式。Objective To explore the application effect of autologous iliac bone transplantation combined with Y-shaped titanium plate internal fixation in sternal reconstruction after sternal manubrium tumor resection.Methods The clinical data of 5 patients with sternal manubrium tumor treated by autologous iliac bone transplantation combined with Y-type titanium plate internal fixation in the thoracic surgery department of Shaanxi provincial people's hospital were retrospectively analyzed.The safety and effectiveness of the operation were evaluated by the occurrence of postoperative complications,such as pulmonary complications,local infection,flap necrosis,prosthesis loosening and abnormal breathing.Results Among the 5 patients in this study,1 patient underwent total sternal resection and 4 patients underwent subtotal sternal resection.Postoperative assessment of respiratory movement,chest shape and lung function were normal.One patient developed lung inflammation,which was cured after anti-inflammatory treatment,no local infection,flap necrosis,prosthesis loosening and abnormal breathing and other complications occurred.Postoperative chest CT showed a good shape of the thorax,and the position of the iliac bone and internal fixation were fixed.One patient underwent internal fixation removal at 26 months after operation,postoperative chest CT showed that the iliac bones and bilateral clavicles and ribs were complete osteogenesis.Conclusion For patients with sternal manubrium tumor,autologous iliac bone transplantation combined with Y-shaped titanium plate internal fixation in sternal reconstruction is a safe and reliable operation.
