摘 要:目的:评价2%氯己定(chlorhexidine,CHX)预处理牙本质表面不同时间对全酸蚀牙本质粘接剂耐久性的影响。方法:将25颗无龋第三磨牙离体牙去除冠部釉质后暴露冠中部牙本质,随机分成5组。对照组按全酸蚀粘接剂使用说明进行酸蚀,粘接及树脂充填步骤。实验组在磷酸酸蚀牙本质后分别用2%CHX处理20 s,40 s,60 s,3 min,然后按照常规步骤继续进行粘接及树脂充填。将样本制备成横截面为0.9 mm×0.9 mm的微拉伸测试试件,检测即刻和冷热循环老化处理后微拉伸强度,扫描电镜下观察牙本质粘接界面的纳米渗漏情况。结果:2%CHX处理后的各组与对照组的即刻粘接力之间没有显著差异(P>0.05)。经5000次冷热循环老化处理后,对照组粘接力明显降低,CHX 20 s,40 s组也有下降,但不如对照组下降明显。CHX处理60 s,3 min两组冷热循环处理后粘接力和即刻粘接力无显著差异(P>0.05)。扫描电镜观察,经过5000次冷热循环老化处理后,各组牙本质粘接界面均存在不同程度的纳米渗漏,但经过CHX处理后的粘接界面纳米渗漏减少。结论:在牙本质粘接过程中,2%氯己定预处理酸蚀后的牙本质表面至少60 s可以有效提高粘接耐久性。Objective:To evaluate the effect of different 2%chlorhexidine(CHX)pretreatment time on the bonding durability of total-etch dental adhesive.Methods:The dentin surface of 25 non-caries third molar was etched by 35%phosphoric acid.Then the partially demineralised surface was treated by CHX for 0 s,20 s,40 s,60 s,3 min.Dental adhesive and resin composite was applied on the dentine surface following the manufacturer's recommendations.Then the specimen was cuted into 0.9 mm×0.9 mm×10 mm sticks.The immediate microtensile bond strength and aging microtensile bond strength of different groups was measured and the nanoleakage was observed by scanning electron microscope(SEM).Results:There was no significant difference between the immediateμTBS of each group.After 5000 times of thermocycles,the bond strength of control group decreased evidently.TheμTBS of group CHX 20 s,40 s decreased,but not as significantly as the control group.There was no significant difference between the immediate and aging bond strength of group CHX 60 s and 3 min(P>0.05).Nanoleakage could be observed in each group,but for CHX treated groups,the nanoleakage was obviously reduced under SEM observation.Conclusion:In order to increase the bonding durability,the etched dentine surface should be treated by 2%chlorhexidine for at least 60 s.
