摘 要:目的探讨儿科护士职业倦怠对代谢性指标的影响和相关性分析。方法选取在湖北省9家三级甲等医院工作的426名儿科护士参加本次调查。所有被调查者建立个人档案,记录基本信息。测量身高、体重和血压,计算体质指数(BMI)。血糖和血脂数据来源于问卷调查1周内的职工健康检查数据。健康检查时间为2019年9—11月。使用职业倦怠量表(MBI-HSS)中文版调查护士职业倦怠情况。结果所有被调查者性别均为女性,平均年龄为(29.5±5.5)岁。所有被调查对象按情感衰竭维度划分,轻度倦怠126人(29.58%),中度倦怠230人(53.99%),重度倦怠70人(16.43%);按去人格化维度划分,轻度倦怠183人(42.96%),中度倦怠168人(39.44%),重度倦怠75人(17.61%);按个人成就感维度划分,轻度倦怠178人(41.78%),中度倦怠205人(48.12%),重度倦怠43人(10.09%)。在情感衰竭维度,不同职业倦怠情况人群BMI、收缩压和总胆固醇(TC)比较,差异均有统计学意义(均P<0.05);在去人格化维度,不同职业倦怠情况人群BMI(P<0.05)、收缩压(P<0.05)和TC差异有统计学意义(均P<0.05)。在情感衰竭维度,职业倦怠评分分别与BMI、收缩压和TC呈正相关(r=0.131、0.145、0.149,均P<0.05);在去人格化维度,职业倦怠评分分别与BMI、收缩压和TC呈正相关(r=0.134、0.135、0.194,均P<0.05)。线性回归模型显示,在情感衰竭和去人格化维度,职业倦怠评分分别与BMI(P<0.05)、收缩压(P<0.05)和TC(P<0.01)存在线性相关。在情感衰竭维度和去人格化维度,职业倦怠均能够正向预测儿科护士的收缩压(γ=0.18,t=3.55,P<0.01;γ=0.16,t=4.22,P<0.01)。结论在情感耗竭和个人成就感维度,职业倦怠能增加湖北省9家三级甲等医院儿科护士BMI、收缩压和TC升高的风险,职业倦怠能正向预测儿科护士的收缩压。Objective To investigate the effect of job burnout on metabolic indexes in pediatric nurses,and analyze the correlation.Methods A total of 426 pediatric nurses working in nine tertiary class A hospitals in Hubei Province participated in the study.All the respondents established a personal file to record basic information.Height,weight and blood pressure were measured and body mass index(BMI)was calculated.The blood glucose and lipid data were from the physical examination data of the employees within one week of the questionnaire survey,and the physical examination was performed from September to November 2019.The Chinese version of the Maslach Burnout Inventory Human Service Survey(MBI-HSS)was used to investigate the job burnout of nurses.Results All respondents are female,with an average age of(29.5±5.5)years old.According to the emotional exhaustion dimension,there were 126 people with mild burnout(29.58%),230 with moderate burnout(53.99%),and 70 with severe burnout(16.43%).According to the depersonalization dimension,there were 183 people with mild burnout(42.96%),168 with moderate burnout(39.44%),and 75with severe burnout(17.61%).According to the the dimension of personal achievement,there were 178 people with mild burnout(41.78%)and 205 with moderate burnout(48.12%),and 43 people had severe burnout(10.09%).In the dimension of emotional exhaustion,the differences in BMI,systolic pressure and total cholesterol(TC)were statistically significant between different job burnout groups(all P<0.05).In the depersonalized dimension,the differences in BMI,systolic pressure and TC were statistically significant between different job burnout groups(all P<0.05).In the dimension of emotional exhaustion,burnout scores were significantly positively correlated with BMI,systolic pressure and TC(r=0.131、0.145、0.149,all P<0.05).In the depersonalized dimension,burnout scores were significantly positively correlated with BMI,systolic pressure and TC(r=0.134、0.135、0.194,all P<0.05).The linear regression model showed that i
