血清维生素D结合蛋白联合胃蛋白酶原-Ⅰ对早期胃癌的诊断价值

摘　　要：目的:探讨血清维生素D结合蛋白(VDBP)联合胃蛋白酶原-Ⅰ(PG-Ⅰ)对早期胃癌的诊断价值。方法:选取2017年10月至2019年10月山西省人民医院消化科收治的108例早期胃癌患者为早期胃癌组,男81例,女27例,年龄(49.34±4.15)岁,年龄范围为35~59岁。另选取同期行身体检查的105例健康者为健康组,男75例,女30例,年龄(50.12±4.31)岁,年龄范围为37~60岁。比较两组的血清VDBP、PG-Ⅰ、胃蛋白酶原比值(PGR)水平及其各个指标对早期胃癌的临床诊断效果。结果:早期胃癌组患者的VDBP[(4.07±1.36)μg/L]、PG-Ⅰ[(88.81±38.67)μg/L]、PGR(6.88±3.33)水平低于健康组[(4.88±1.58)μg/L、(105.2±43.09)μg/L、(8.43±4.17)],差异有统计学意义(P<0.05);Pearson相关性分析显示VDBP与PG-Ⅰ、PGR水平呈显著正相关;ROC曲线结果显示VDBP、PG-Ⅰ、PGR诊断早期胃癌患者的曲线下面积分别为0.878、0.793、0.833;联合诊断后VDBP/PG-Ⅰ诊断的曲线下面积为0.895,VDBP/PGR诊断的曲线下面积为0.916。结论:VDBP可作为筛查早期胃癌患者的新型诊断指标,与PG-Ⅰ联合诊断可进一步提高对早期胃癌筛查的准确度。Objective To investigate the clinical significance of serum vitamin D binding protein(VDBP)combined with pepsinogen-Ⅰ(PG-Ⅰ)in the diagnosis of patients with early gastric cancer.Methods A retrospective study was performed on 108 cases of patients with early gastric cancer who were admitted in the department of Gastroenterology,Shanxi Provincial People′s Hospital from October 2017 to October 2019.The patients were divided into the early gastric cancer group,including 81 males and 27 females,aged(49.34±4.15)years old,ranging from 35 to 59 years old.In addition,105 healthy subjects included in the department of Gastroenterology during the same period were selected as the healthy group,including 75 males and 30 females,aged(50.12±4.31)years old,ranging from 37 to 60 years old.Two groups of serum VDBP,PG-Ⅰ,pepsinogen ratio(PGR)level and the effect of the clinical diagnosis of early gastric cancer were compared.Results The levels of VDBP[(4.07±1.36)μg/L],PG-Ⅰ[(88.81±38.67)μg/L]and PGR(6.88±3.33)in early gastric cancer group were lower than those in healthy group[(4.88±1.58)μg/L,(105.2±43.09)μg/L,(8.43±4.17)],and the differences were statistically significant(P<0.05);Pearson correlation analysis showed that VDBP was positively correlated with PG-Ⅰand PGR;ROC curve showed that the area under the curve of VDBP,PG-Ⅰand PGR were 0.878,0.793 and 0.833;the area under the curve of VDBP/PG-Ⅰand VDBP/PGR were 0.895 and 0.916.Conclusion VDBP can be used as a new diagnostic index for screening patients with early gastric cancer,and combined diagnosis with PG-Ⅰcan further improve the accuracy of screening for early gastric cancer.
