摘  要:目的研究乙酰左旋肉碱对大鼠急性脊髓损伤后的干预效果,从行为学、组织形态学和线粒体结构探讨乙酰左旋肉碱对大鼠运动功能恢复的影响。方法将大鼠随机分为脊髓损伤组、乙酰左旋肉碱治疗组、假手术组3组,每组22只,构建大鼠脊髓损伤模型。于损伤后1 d、3 d、7 d、14 d、21 d、28 d和35 d评估大鼠后肢运动功能恢复情况;并取损伤后1 d、3 d的脊髓组织行HE染色,进行形态学观察;透射电镜观察脊髓损伤后24 h线粒体形态的变化。结果Basso Beattie Bresnahan运动功能评分损伤后7 d、14 d、21 d、28 d、35 d,乙酰左旋肉碱组评分均高于SCI组,差异有统计学意义(P<0.05);SCI+ALC组损伤后3 d,变性的神经元细胞结构逐渐清楚,细胞核清晰,核膜完整;神经元胞浆内线粒体含量较多且结构完整,线粒体空泡化也较损伤组减轻。结论乙酰左旋肉碱可能通过改善脊髓神经元线粒体功能,促进大鼠脊髓损伤后后肢运动功能的恢复。Objective To study the intervention effect of acetyl-L-carnitine on acute spinal cord injury in rats,and to explore the effect of acetyl-L-carnitine on the recovery of motor function in rats from the aspects of behavior,histomorphology and mitochondrial structure.Methods The spinal cord injury model of SD rats was established and randomly divided into three groupsSham operation group,spinal cord injury group and acetyl L-carnitine treatment group with 22 rats in each group.The recovery of motor function of hindlimb was evaluated at 1 day,3 days,7 days,14 days,21 days,28 days and 35 days after injury,and the spinal cord tissue at 1 day and 3 days after injury was stained with HE for morphological observation,and the morphological changes of mitochondria at 24 hours after spinal cord injury were observed by transmission electron microscope.Results On the 7th,14th,21st,28th and 35th day after injury,the score of Basso Beattie Bresnahan motor function in the acetyl-L-carnitine group was higher than that in the SCI group,and the difference was statistically significant 3 days after injury in the SCI+ALC group,the structure of the denatured neurons was gradually clear,the nucleus was clear and the nuclear membrane was intact,the content of mitochondria in the cytoplasm of neurons was more and intact,and the vacuolation of mitochondria was less than that in the injury group.Conclusion Acetyl L-carnitine may promote the recovery of hindlimb motor function after spinal cord injury through the mitochondrial function of spinal cord neurons.
