摘 要:目的分析非酒精性脂肪肝(NAFL)并发肝功能损害的相关危险因素,为改善NAFL防治策略提供依据。方法于2019年8月对260例NAFL患者进行回顾性病例调查,其中128例并发肝功能损害患者作为观察组,132例未并发肝功能损害患者作为对照组,采用单因素及Logistic多因素分析NAFL并发肝功能损害的相关危险因素。结果 Logistic多因素分析显示,肥胖(OR=3.691)、饮酒(OR=5.364)、高血压(OR=3.522)、吸烟(OR=2.721)、LDL-C(OR=1.699)、FBG(OR=1.625)及UA(OR=1.584)水平升高是NAFL并发肝功能损害发生的危险因素(P<0.05);HDL-C(OR=0.043)水平升高及经常锻炼(OR=0.287)为NAFL并发肝功能损害的保护因素(P<0.05)。结论饮酒、吸烟、高血压、肥胖、缺乏锻炼及LDL-C、FBG、UA和HDL-C水平升高是诱发NAFL并发肝功能损害的危险因素,合理膳食、经常锻炼有助于预防和延缓病情的进一步发展。Objective To analyze the relative risk factors of non-alcoholic fatty liver(NAFL) complicated with liver function damage.In order to provide the evidence for improving prevention and control strategies of NAFL.Methods A retrospective case study was conducted among 260 NAFL patients in August 2019, 128 patients complicated with liver function damage were included in the observation group and 132 patients without liver function damage were included in the control group. The relative risk factors of NAFL complicated with liver function damage were analyzed by single factor and multivariate logistic analysis. Results Multivariate logistic analysis showed that obesity(OR=3.691), chronic drinking(OR=5.364), hypertension(OR=3.522), smoking(OR=2.721), LDL-C(OR=1.699), FBG(OR=1.625) and higher UA level(OR=1.584) were the risk factors of NAFL complicated with liver function damage(P<0.05). Higher HDL-C level(OR=0.043)and regular exercises(OR=0.287)were the protective factors of NAFL complicated with liver function damage(P<0.05). Conclusion Drinking alcohol, smoking, high blood pressure, obesity, lack of exercises and increased levels of LDL-C, FBG, UA and HDL-C are the risk factors of NAFL complicated with liver function damage, reasonable diet and regular exercises are helpful to prevent and delay future development of the disease.
