心理干预联合盐酸帕罗西汀治疗产后抑郁症对照研究

摘　　要：目的探讨心理干预联合盐酸帕罗西汀治疗对产后抑郁的临床治疗效果。方法选取产后抑郁患者92例,根据患者自愿原则分成观察组(40例)和对照组(52例),观察组采用盐酸帕罗西汀加心理治疗,对照组只采用盐酸帕罗西汀治疗,共治疗8周。于治疗前和治疗后第2、4、6、8周末对两组患者进行爱丁堡产后抑郁量表(EPDS)及汉密尔顿抑郁量表(HAMD)测评,采用治疗中需处理的不良反应症状量表(TESS)评定不良反应。结果治疗8周后,观察组的治疗有效率高于对照组(P<0.05)。治疗后第6、8周末,两组HAMD、EPDS评分均较各自治疗前降低,且观察组HAMD、EPDS评分均低于对照组(P<0.05)。两组总不良反应发生率比较差异无统计学意义(P>0.05)。结论心理干预联合盐酸帕罗西汀治疗产后抑郁临床效果显著,优于单纯药物治疗,值得推荐应用。Objective To explore the clinical effect of psychological intervention combined with paroxetine drug therapy on postpartum depression.Methods A total of 92 patients with postpartum depression were selected and divided into observation group(40 cases)treated with psychological intervention combined with paroxetine drug therapy and control group(52 cases)treated only with paroxetine drug therapy according to the patients’willingness for 8 weeks.They were assessed with Edinburgh Depression Scale(EPDS)and Hamilton Depression Scale(HAMD)before treatment and at the end of the 2^nd,4^th,6^th and 8^th week after treatment.Treatment Emergent Symptoms Scale(TESS)was used to evaluate adverse reactions.Results At the end of the 8^th week,the response rate in observation group was significantly higher than that in control group(P<0.05).At the 6th and 8th weekends after treatment,the total scores of HAMD and EPDS in both groups decreased significantly compared with those before treatment(P<0.05),and the total scores of HAMD and EPDS in observation group were significantly lower than those in control group(P<0.05).There was no significant difference in the incidence rates of adverse reactions between two groups(P>0.05).Conclusion Psychological intervention combined with paroxetine drug therapy for postpartum depression has significant clinical effect,which is superior to drug therapy alone,and is worthy of recommendation.
