3D与2D腹腔镜下肠粘连松解术的临床疗效对比分析

摘　　要：目的:探讨3D与2D腹腔镜下肠粘连松解术的临床疗效。方法:回顾分析2015年11月至2019年2月为64例患者行腹腔镜下肠粘连松解术的临床资料,其中31例行3D腹腔镜手术(3D组),33例行2D腹腔镜手术(2D组),对比两组围手术期及手术并发症情况。结果:两组中转开腹率、术后肛门恢复排气时间、术后住院时间、住院费用、术中肠管浆肌层损伤率、术后并发症发生率差异无统计学意义(P>0.05);3D组手术时间、术中出血量优于2D组,差异有统计学意义(P<0.05)。结论:3D腹腔镜手术行肠粘连松解术可降低手术操作难度,缩短手术时间,减少术中出血,而且不增加住院费用,较2D腹腔镜具有一定优势。Objective:To investigate the clinical efficacy of 3D laparoscopic enterolysis and 2D laparoscopic enterolysis.Method:The clinical data of 64 patients who underwent laparoscopic enterolysis between Nov.2015 and Feb.2019 were retrospectively analyzed.Among them,31 patients underwent 3D laparoscopic surgery(3D group)and 33 patients underwent 2D laparoscopic surgery(2D group).The perioperative situation and the occurrence of surgical complications of the two groups were compared.Results:The conversion rate,the postoperative anal recovery exhaust time,the postoperative hospitalization time and the hospitalization expenses of the two groups were not significantly different,there was no significance(P>0.05).The incidence of intraoperative intestinal seromuscular layer injury and the incidence of postoperative complications of the two groups were not significantly different(P>0.05).Compared with the 2D Group,the 3D Group could shorten the operation time and reduce the bleeding during the operation,the difference was statistically significant(P<0.05).Conclusions:The application of 3D laparoscopic system for enterolysis could reduce the difficulty of operation,shorten the operation time,reduce the bleeding during the operation,and may reduce the incidence of surgical complications without increasing hospitalization expenses.Compared with 2D laparoscopic system,3D laparoscopic system has certain advantages and is worthy of clinical promotion.
