术前短期肺康复训练对肺癌患者围手术期心肺功能及并发症的影响

摘　　要：目的:观察术前短期肺康复训练对肺癌合并手术高危因素患者心肺功能及术后并发症的影响。方法:选取自2018年12月-2019年10月在中山市人民医院胸外科手术治疗的64例肺癌合并手术高危因素患者,按随机数字表法分成康复组和对照组各32例。在常规术前准备相同的情况下,康复组患者术前给予1次/d、30 min/次、强度为60%~80%VO2max的踏车训练,配合呼吸训练,连续7 d,比较两组患者术前心肺功能和术后并发症发生率。结果:肺康复训练后,康复组在术前FVC、MVV、PEF、VO2max、VO2/kg、AT、O2pulse方面较对照组有明显提高,差异均有统计学意义(P<0.05);术后康复组肺部并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:术前短期肺康复训练可以提高肺癌合并高危因素患者术前的心肺功能,降低术后肺部并发症发生率。Objective:To investigate the effect of preoperative short-term rehabilitation training on cardiopulmonary function and postoperative complications in patients with high risk factors who are eligible for lung cancer surgery.Method:From December 2018 to October 2019,64 lung cancer patients with high risk factors in cardiothoracic surgery department of Zhongshan People’s Hospital were randomly divided into the rehabilitation group(n=32)and the control group(n=32).Both groups received routine preoperative preparation,and the rehabilitation group received treadwheel training(once a day,30 minutes each time,60%-80%VO2 max level)and respiratory training for 7 days.The differences of preperative cardiopulmonary function and the incidence of postoperative complications were compared between the two groups.Result:After preoperative rehabilitation training,FVC,MVV,PEF,VO2max,VO2/kg,AT,O2pulse in the rehabilitation group were improved compared with those in the control group,the differences were statistically significant(P<0.05).The incidence of postoperative pulmonary complications of rehablitation group was significantly lower than that of control group(P<0.05).Conclusion:The preoperative short-term rehabilitation training can improve cardiopulmonary function in lung cancer patients with high risk factors,and reduce the incidence of postoperative complications.
