血清CYFRA21-1和EBV相关抗体联合检测对鼻咽癌的诊断价值

摘　　要：目的:探讨血清细胞角蛋白19片段(Cytokeratin-19-fragment,CYFRA21-1)和和EB病毒壳抗原免疫球蛋白A抗体(Epstein-barr viral capsid antigen immunoglobulin A,EB-VCA-IgA)联合检测对鼻咽癌的诊断价值。方法:选取2016年5月至2018年6月期间我院收治的27例鼻咽癌患者(研究组)和同期在我院进行健康体检的27例健康者(对照组)为研究对象。分别采用电化学发光免疫分析技术和免疫酶技术对血清中的CYFRA21-1和EB-VCA-IgA水平进行测定。结果:研究组患者血清CYFRA21-1水平和阳性率、EB-VCA-IgA滴度测量值和阳性率均高于对照组(P<0.05);CYFRA21-1和EB-VCA-IgA联合检测的敏感性和特异性分别为98.3%和95.6%,高于CYFRA21-1单独检测(48.1%、76.5%)和EB-VCA-IgA单独检测(77.3%、69.6%)(P<0.05)。结论:联合CYFRA21-1和EB-VCA-IgA作为辅助检查检测诊断鼻咽癌的特异性和敏感性都较高,有助于提高鼻咽癌的临床确诊率。Objective:To explore the diagnostic value of combined detection of CYFRA21-1 and EBV related antibodies in nasopharyngeal carcinoma(NPC).Methods:From May 2016 to June 2018,27 NPC patients(Study Group)admitted to’and 27 healthy people(control group)who had physical examination in our hospital during the same period were selected as the study objects.The serum concentrations of CYFRA21-1 and EB-VCA-IgA in were measured by electrochemiluminescence immunoassay and immunoenzyme technique respectively.Results:The serum CYFRA21-1 concentration and positive rate,EB-VCA-IgAconcentration and positive rate in the study group were higher than those in control group(P<0.05).The sensitivity and specificity of CYFRA21-1 combined withEB-VCA-IgAin diagnosing NPC were 98.3%and 95.6%,higher than those of CYFRA21-1(48.1%,76.5%)and EB-VCA-IgA(77.3%,69.6%)(P<0.05).Conclusion:The combination of CYFRA21-1 and EB-VCA-IgA as the auxiliary examination has high specificity and sensitivity in the diagnosis of NPC,which is helpful to improve the clinical diagnosis.
