通窍定眩汤联合甲磺酸倍他司汀治疗痰瘀互阻型后循环缺血性眩晕的临床观察

摘　　要：目的:分析通窍定眩汤联合甲磺酸倍他司汀治疗痰瘀互阻型后循环缺血性眩晕的临床效果。方法:纳入后循环缺血性眩晕患者60例,采用随机数字表法分为观察组和对照组,各30例。对照组服用甲磺酸倍他司汀,观察组在对照组基础上口服通窍定眩汤,14 d为1个疗程。比较两组治疗前后临床效果,采用TCD检测椎-基底动脉血流速度(基底动脉平均血流速度、收缩末期血流速度、舒张末期血流速度),观察不良反应情况。结果:观察组治疗后基底动脉平均血流速度、收缩末期血流速度及舒张末期血流速度均高于对照组(P<0.05);观察组总有效率高于对照组(P<0.05);两组均未出现不良反应,差异无统计学意义(P>0.05)。结论:通窍定眩汤联合甲磺酸倍他司汀治疗痰瘀互阻型后循环缺血性眩晕的临床效果确切,可改善患者的椎-基底动脉血流速度,对血、尿、便常规、肝肾功能及心电图无不良影响。Objective:To analyze the clinical effect of Tongqiao Dingxuan Decoction combined with Betahistine Mesilate in treating posterior circulation ischemia vertigo with phlegm and blood stasis.Method:Sixty patients with posterior circulation ischemic vertigo were included.They were divided into the observation group and the control group by random number table,30 cases in each group.The control group was treated with Betahistine Mesilate,and the observation group was orally Tongqiao Dingxuan Decoction on the basis of the control group,and 14 days was one course of treatment.The clinical efficacy of the two groups was compared before and after treatment,and the vertebral-basal artery blood flow velocity(average basilar artery blood flow velocity,end-systolic blood flow velocity and end-diastolic blood flow velocity)was measured by TCD,and the adverse reactions were observed.Result:The average basilar artery blood flow velocity,end-systolic blood flow velocity and end-diastolic blood flow velocity in the observation group after treatment were higher than those of the control group(P<0.05).The total effective rate in the observation group was higher than that of the control group(P<0.05).There was no adverse reaction between the two groups,and the difference was not statistically significant(P>0.05).Conclusion:Tongqiao Dingxuan Decoction combined with Betahistine Mesilate has a definite clinical effect on the treatment of posterior circulation ischemic vertigo with phlegm and blood stasis.It can improve the vertebral-basal artery blood flow velocity of patients and has no adverse effects on the blood,urine,stool routine,liver and kidney function and electrocardiogram.
