摘　　要：目的探讨胰岛素瘤相关蛋白1(INSM1)在细胞学样本中的表达及其鉴别诊断价值。方法应用免疫细胞化学En Vision两步法检测INSM1、Syn、Cg A、CD56在肺神经内分泌癌(26例),肺腺癌(55例),淋巴造血系统肿瘤(11例)及间皮细胞增生(3例)细胞蜡块样本中的表达。结果①26例神经内分泌癌中男性19例,女性7例,年龄范围49~71岁,中位年龄65.5岁。②INSM1、Syn、Cg A、CD56在肺神经内分泌癌细胞蜡块中的阳性率分别为100%、100%、57.7%和100%,显著高于肺腺癌、淋巴造血肿瘤及间皮细胞增生的阳性率(P<0.01)。③INSM1在肺神经内分泌癌细胞蜡块中的敏感性显著高于Cg A(P<0.01),与Syn和CD56无显著性差异(P>0.05);INSM1在肺神经内分泌癌细胞蜡块诊断中的特异性为96.4%,显著高于CD56(81.8%)(P<0.05),而与Syn和Cg A无显著性差异(P>0.05)。④与53例进行活检的组织学标本进行对比,INSM1免疫组化染色在细胞蜡块和组织学检测结果具有高度一致性(κ=1)。结论INSM1免疫组化染色在细胞蜡块样本神经内分泌癌鉴别诊断中非常可靠。Objective To detect the diagnostic performance of INSM1 in neuroendocrine tumors in cytological specimens.Methods The expression of INSM1,Syn,Cg A,and CD56 was immunostained by En Vision two-step methods in 26 cases of lung neuroendocrine carcinoma,55 cases of lung adenocarcinoma and 10 cases of lymphoma.Results Among the 95 cases of neuroendocrine tumors,19 patients were male and 7 were female,the age ranged from 49 to 71 years(median age 65.5 years).The positive rate of INSM1,Syn,Cg A and CD56 for neuroendocrine tumors was 100%,100%,57.7%and 100%,respectively,which was significantly higher than any other types of samples(P<0.01).For neuroendocrine tumors,the sensitivity of INSM1 was significantly higher than Cg A(P<0.01),similar to Syn and CD56(P>0.05);the specificity was significantly higher than CD56(P<0.05),similar to Syn and Cg A(P>0.05).INSM1 showed perfect agreement between biopsy and cytoblock specimens of the 53 cases(κ=1).Conclusion INSM1 is a reliable marker of neuroendocrine tumors in cytoblock specimens.
