乳腺实性乳头状癌23例临床与病理分析

摘　　要：目的探讨实性乳头状癌的临床特点及组织学形态、免疫组织化学结果等。方法回顾性分析23例乳腺实性乳头状癌患者的临床资料,结合组织学形态及免疫组织化学结果,分析其临床病理特点。结果患者均为女性,发病年龄31~80岁,平均63岁。临床特征表现为乳腺肿块及乳头溢血。21例单侧乳腺发生,2例双侧乳腺发生。共获得乳腺肿块25例,其中原位实性乳头状癌9例;伴有微浸润的2例;伴有浸润性导管癌或黏液癌的有14例。其组织学形态大部分为巢团状结构,个别为导管内乳头状结构,大部分病例可见纤细的纤维血管轴心,细胞形态较温和,核分裂少见。免疫组化:ER强(+),PR强弱程度不一;82%的病例Syn(+);73%的病例Cg A(+);56%的病例CD56(+)。结论乳腺实性乳头状癌好发于老年女性,少数年轻患者亦可发生,其病理特征和免疫表型较为独特,随访资料显示实性乳头状癌具有较好的预后。Objective To investigate the clinicopathologic features of solid papillary carcinoma of the breast.Methods Histopathological characteristics and immunohistochemical markers were studied retrospectively and relevant literatures were reviewed.Results All the patients were females with a mean age of 63 years.Some patients presented as breast masses or nipple bloody discharge.9 cases were diagnosed as carcinoma in situ and 2 cases with minimally invasive and 14 cases with invasive.Histologically,all the tumor cells showed a solid nest structure and the cells were moderate and uniform.Immunophenotypically,all cases were strongly positive for ER and most were different positive for PR.Most cases were positive for chromogranin A(82%)and synaptophysin(73%)and CD56(56%).Conclusions SPC predominantly affects elderly females and has distinctive pathologic features and immunophenotype.Pathologic features and follow-up data suggest that SPC has favorable prognosis.
