摘 要:目的:探讨幽门螺杆菌(Hp)感染与慢性阻塞性肺疾病(COPD)患者炎症反应、肺功能和血气指标的关系。方法:选取安徽医科大学附属宿州医院呼吸内科2017年3月至2019年7月收治的80例COPD患者(观察组)和同期80例体检健康者(对照组)作为研究对象,检测两组Hp感染情况,将观察组分为Hp阳性组(n=50)和Hp阴性组(n=30),对比Hp阳性组和HP阴性组临床资料、炎性因子[C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、IL-8]、肺功能指标[第1秒用力呼气量(FEV1)、FEV1占用力肺活量比值(FEV1/FVC)、FEV1占预计值百分数(FEV1/pred)]、血气指标[动脉氧分压(PaO2)、动脉二氧化碳分压(PaCO2)、血氧饱和度(SaO2)]的差异。结果:观察组Hp感染率高于对照组(P<0.05)。观察组Hp感染率随着肺气肿程度的加重逐渐升高(P<0.05)。HP阳性组CRP、TNF-α、IL-6、IL-8、PaCO2均高于HP阴性组和对照组(P<0.05),FEV1/pred、FEV1/FVC、PaO2、SaO2均低于HP阴性组和对照组(P<0.05)。结论:COPD患者存在较高的Hp感染率,Hp感染与COPD患者肺气肿程度有关,其可加重COPD炎症反应和低氧血症,并降低肺功能。Objective:To investigate the relationship between Helicobacter pylori(Hp)infection and inflammation,lung function and blood gas indices in patients with chronic obstructive pulmonary disease(COPD).Methods:In this study,80 COPD patients(observation group)and 80 healthy persons(control group)who were admitted to the department of respiratory medicine of Suzhou Hospital Affiliated to Anhui Medical University from March 2017 to July 2019 were selected as the research subjects.The observation group were divided into Hp positive group(n=50)and Hp negative group(n=30)according to the result of Hp infection test.The Hp positive and negative groups were compared in terms of clinical data,inflammatory factors including C reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and IL-8,lung function indices including forced expiratory volume in the first second(FEV1),proportion of FEV1 to forced vital capacity(FEV1/FVC),percentage of predicted FEV1 value(FEV1/pred)and blood gas indices including partial pressure of oxygen(PaO2),partial pressure of carbon dioxide(PaCO2)and blood-oxygen saturation(SaO2).Results:The Hp infection rate in the observation group was higher than that in the control group(P<0.05).The Hp infection rate in the observation group gradually increased with the aggravation of emphysema(P<0.05).CRP,TNF-α,IL-6,IL-8,PaCO2 in HP positive group were higher those in HP negative group and control group(P<0.05).FEV1/pred,FEV1/FVC,PaO2 and SaO2 in HP positive group were all lower than those in HP negative group and control group(P<0.05).Conclusion:COPD patients have a higher rate of Hp infection.Hp infection is related to the degree of emphysema in COPD patients.It can aggravate COPD inflammation and hypoxemia and reduce lung function.
