摘　　要：【目的】探讨全胸腔镜肺叶切除对老年肺癌患者蛋白代谢反应指标及应激因子的影响。【方法】回顾性选择2016年4月至2018年4月在本院诊治的120例肺癌患者,根据手术方案不同分为观察组和对照组。观察组采用全胸腔镜肺叶切除术,对照组采用传统开胸肺叶切除术。比较两组手术情况及手术前、术后5 d血清蛋白代谢反应指标视黄醇结合蛋白(RBP)、前清蛋白(PA)、转铁蛋白(TRF)和疼痛应激因子缓激肽(BK)、P物质(SP)、前列腺素E2(PGE2)水平。【结果】观察组切口长度、出血量、手术时间、住院时间均明显少于对照组,其差异有统计学意义(P<0.05)。术后,观察组血清RBP、PA、TRF水平降低幅度小于对照组,血清BK、SP、PGE2水平上升幅度小于对照组,其差异均有统计学意义(P<0.05)。【结论】采用全胸腔镜肺叶切除术治疗老年肺癌患者,可降低患者的疼痛应激程度,对代谢反应的影响相对较小,是一种安全有效的治疗方法,值得临床推广应用。【Objective】To investigate the effect of total thoracoscopic lobectomy on protein metabolism and stress factors in elderly patients with lung cancer.【Methods】Methods:a total of 120 patients with lung cancer who were treated in our hospital from April 2016 to April 2018 were retrospectively selected and divided into observation group and control group according to different surgical schemes.The observation group was treated with thoracoscopic lobectomy,and the control group was treated with traditional thoracotomy lobectomy.Before and 5 days after operation,the serum protein metabolic reaction indexes such as retinol binding protein(RBP),prealbumin(PA),transferrin(TRF),bradykinin(BK),substance P(SP)and prostaglandin E2(PGE2)were compared between the two groups.【Results】The incision length,blood loss,operation time and hospitalization time in the observation group were significantly less than those in the control group,and the difference was statistically significant(P<0.05).After operation,the decrease range of serum RBP,PA,TRF levels in the observation group was less than that in the control group,and the increase range of serum BK,SP,PGE2 levels was less than that of the control group,the differences were statistically significant(P<0.05).【Conclusion】Thoracoscopic lobectomy in the treatment of elderly patients with lung cancer can reduce the degree of pain and stress in patients with relatively small impact on metabolic response,which is a safe and effective treatment method,worthy of clinical application.
