腹腔镜右半结肠切除术对结肠癌患者出血量与排气时间的影响

摘　　要：目的:探讨腹腔镜右半结肠切除术对结肠癌患者出血量与排气时间的影响。方法:选取2018年4月-2020年3月笔者所在医院收治的结肠癌患者92例作为研究对象,采用信封法分为两组,对照组行传统经腹右半结肠切除术治疗,研究组行腹腔镜右半结肠切除术治疗。对比两组临床指标、术后并发症发生率、生活质量、白细胞计数及炎性因子水平。结果:研究组手术耗时、排气时间、术后住院时间均短于对照组,术中出血量少于对照组(P<0.05);研究组术口感染、肠梗阻、吻合口瘘、肺栓塞、腹腔感染发生率为13.0%,低于对照组的30.4%(P<0.05);两组治疗前躯体角色、生命活力、机体健康、疼痛等生活质量评分比较,差异均无统计学意义(P>0.05);研究组治疗后生活质量各项评分均高于对照组(P<0.05);两组治疗前WBC、CRP、TNF-α水平比较,差异均无统计学意义(P>0.05);治疗后72 h,两组WBC、CRP、TNF-α水平均较治疗前升高,但研究组均低于对照组(P<0.05)。结论:对结肠癌患者应用腹腔镜右半结肠切除术治疗,可减少术中出血量,促进早期排气,降低术后并发症发生率,减轻炎性反应,提升患者生活质量,可考虑推广。Objective:To investigate the effect of laparoscopic right hemicolectomy on the amount of bleeding and exhaust time in patients with colon cancer.Method:A total of 92 patients with colon cancer admitted to our hospital from April 2018 to March 2020 were selected as study subjects and divided into two groups by envelope method.The control group was treated with traditional transabdominal right hemicolectomy and the study group was treated with laparoscopic right hemicolectomy.The clinical indexes,incidence of postoperative complications,quality of life,white blood cell count and inflammatory factors level were compared between the two groups.Result:The operation time,exhaust time and postoperative hospital stay in the study group were shorter than those of the control group,and the amount of bleeding during operation was less than that of the control group(P<0.05).The incidence of intraoperative infection,intestinal obstruction,anastomotic fistula,pulmonary embolism and abdominal infection in the study group was 13.0%,which was lower than 30.4%of the control group(P<0.05).The quality of life scores of somatic role,life vitality,body health and pain before treatment were compared between the two groups,and the differences were not statistically significant(P>0.05).The quality of life scores of the study group after treatment were higher than those of the control group(P<0.05).The levels of WBC,CRP and TNF-αbefore treatment were compared between the two groups,and the differences were not statistically significant(P>0.05).At 72 h after treatment,the levels of WBC,CRP and TNF-αin both groups were higher than those before treatment,but those in the study group were lower than those of the control group(P<0.05).Conclusion:Laparoscopic right hemicolectomy for colon cancer patients can reduce the amount of bleeding during operation,promote early exhaust,reduce the incidence of postoperative complications,reduce the inflammatory response,improve the quality of life of patients,which can be considered for promotion.
