腋窝双向淋巴示踪在乳腺癌腋窝淋巴结清扫术中的应用

摘　　要：目的:分析腋窝双向淋巴示踪在乳腺癌腋窝淋巴结清扫术中的应用效果。方法:选择2018年1月-2019年12月笔者所在医院收治的40例乳腺癌患者作为研究对象,采用随机数字表法分为对照组和研究组,每组20例。对照组采用单纯腋窝淋巴结清扫术治疗,研究组在腋窝淋巴结清扫术中增加腋窝双向淋巴示踪技术,对比两组ARM淋巴结检出成功率、腋窝淋巴结完全清除率、术中出血量、术后上肢水肿发生率、住院时间。结果:研究组ARM淋巴结检出成功率、腋窝淋巴结完全清除率均显著高于对照组,术中出血量少于对照组,术后上肢水肿发生率低于对照组,住院时间短于对照组,差异均有统计学意义(P<0.05)。结论:在乳腺癌腋窝淋巴结清扫术中应用亚甲蓝腋窝双向淋巴示踪技术,不仅能提高ARM淋巴结检出成功率和腋窝淋巴结完全清除率,还能降低术后患肢水肿发生率和缩短预后周期,从而全面改善患者预后。Objective:To analyze the effect of two-way axillary lymph tracing in axillary lymphadenectomy of breast cancer.Method:A total of 40 breast cancer patients admitted to our hospital from January 2018 to December 2019 were selected as the research objects.They were divided into the control group and the study group with random number table method,20 cases in each group.The control group was treated with simple axillary lymphadenectomy.The study group added two-way axillary lymph tracing technology in axillary lymphadenectomy,and the success rate of ARM lymph nodes detection,complete clearance rate of axillary lymph nodes,intraoperative bleeding volume,incidence of postoperative upper extremity edema,hospital stay were compared between the two groups.Result:The success rate of ARM lymph nodes detection and the complete clearance rate of axillary lymph nodes in the study group were significantly higher than those of the control group,and the intraoperative bleeding volume was significantly less than that of the control group,and the incidence of postoperative upper extremity edema was significantly lower than that of the control group,and the hospital stay was significantly shorter than that of the control group,and the differences were statistically significant (P<0.05).Conclusion:The application of two-way axillary lymph tracing with Methylene Blue in axillary lymphadenectomy of breast cancer can not only improve the success rate of ARM lymph nodes detection and complete clearance rate of axillary lymph nodes,but also reduce the incidence of postoperative upper extremity edema and shorten the prognosis period,so as to comprehensively improve the prognosis.
